摘 要:目的采用经会阴盆底超声探讨会阴侧切对女性产后前盆腔功能的影响。方法选取经阴道分娩的单胎初产妇162例,其中会阴侧切(A组)94例,非会阴侧切(B组)68例,均于产后6~8周进行盆底超声检查,获得静息状态、最大Valsalva状态时盆底图像数据,测量与分析如下参数:膀胱颈位置、膀胱颈移动度、有无膀胱膨出、有无尿道内口漏斗形成、肛提肌裂孔面积。结果会阴侧切组和非会阴侧切组在静息状态下膀胱颈位置差异无统计学意义(P>0.05)。会阴侧切组和非会阴侧切组的最大Valsalva状态时膀胱颈位置、膀胱颈移动度、肛提肌裂孔面积、膀胱膨出率和尿道内口漏斗形成率差异均无统计学意义(P>0.05)。结论经阴道分娩时,是否行会阴侧切术对产后盆底功能的影响无明显差异,因此会阴侧切不能保护初产妇产后盆底功能。Objective To evaluate the effect of episiotomy on the postpartum female anterior pelvic compartment function by transperineal pelvic floor ultrasound.Methods One hundred and sixty-two singleton primiparas via trmsvaginal delivery were divided into episiotomy group(n=94,group A)and non-episiotomy group(n=68,group B).The parameters of resting and Valsalva condition were measured by the transperineal pelvic floor ultrasound in 6~8 weeks after vaginal birth.The following parameters were measured and analyzed bladder neck position,bladder neck descent,the existence of vesicocele and internal urethral orifice funnel,and levator hiatus area.Results There was no significant difference in the position of the bladder neck between the episiotomy group and the non-episiotomy group at rest(P>0.05).There were no significant differences in the position of bladder neck,bladder neck descent,levator hiatus area,the rate of vesicocele and the incidence rate of internal urethral orifice funnel in the Valsalva state between two groups(P>0.05).Conclusion Episiotomy is no significant difference with pelvic floor function injury during vaginal delivery,so episiotomy cannot protect the postpartum pelvic floor function of primiparas.
