复合保温干预结合改良“人字形”头低足高位对腹腔镜妇科肿瘤切除术中患者应激反应、舒适度的影响

摘　　要：目的探讨复合保温干预结合改良“人字形”头低足高位对腹腔镜妇科肿瘤切除术中患者应激反应、舒适度的影响。方法选取2018年10月至2020年10月收治的150例择期行腹腔镜妇科肿瘤切除术的患者,以抽签法按照单双号分组形式随机将其分为对照组和观察组,各75例。对照组采用常规手术室护理,观察组在对照组护理基础上应用复合保温干预结合改良“人字形”头低足高位干预。比较两组的护理效果。结果气腹后、术毕,观察组的E、NE水平均低于对照组(P<0.05)。观察组OPCQ量表中心理维度、生理维度、环境维度评分及总分均高于对照组(P<0.05)。观察组眼睑水肿、肩部疼痛、下肢肿胀、下肢麻木、下肢疼痛、低体温、寒战发生率均低于对照组(P<0.05)。结论复合保温干预结合改良“人字形”头低足高位干预应用于腹腔镜妇科肿瘤切除术患者手术室护理中能够有效减轻术中的应激反应,提高术中舒适度,减少并发症,值得推广。Objective To explore the effect of compound thermal insulation intervention combined with improved"herringbone"head low and foot high position on stress response and comfort of patients during laparoscopic gynecological tumor resection.Methods A total of 150 patients who underwent elective laparoscopic gynecological tumor resection admitted from October 2018 to October 2020 were selected and randomly divided into control group and observation group according to the odd and even number grouping form of drawing method,with 75 cases in each group.The control group was given routine operating room nursing,and the observation group was given compound thermal insulation intervention combined with improved"herringbone"head low and foot high position intervention on the basis of the nursing of the control group.The nursing effects of the two groups were compared.Results After pneumoperitoneum and operation,the levels of E and NE in the observation group were lower than those in the control group(P<0.05).The scores of psychological dimension,physiological dimension,environmental dimension and total score of OPCQ scale in the observation group were higher than those in the control group(P<0.05).The incidences of eyelid edema,shoulder pain,lower limb swelling,lower limb numbness,lower limb pain,hypothermia and shivering in the observation group were lower than those in the control group(P<0.05).Conclusion In the nursing of patients undergoing laparoscopic gynecological tumor resection,the compound thermal insulation intervention combined with improved"herringbone"head low and foot high position intervention can effectively alleviate the intraoperative stress response,improve intraoperative comfort and reduce complications,and which is worthy of promotion.
