摘 要:目的对从皮肤软组织感染(SSTI)患者中分离的耐甲氧西林金黄色葡萄球菌(MRSA)进行葡萄球菌染色体mec盒(SCC mec)分型和同源性分析,并了解其耐药情况,为规范合理使用抗生素和医院感染控制提供实验室依据。方法收集从SSTI患者中分离的44株MRSA,采用药敏纸片扩散(K-B)法和Vitek 2 Compact仪器法进行抗菌药物敏感性试验,采用普通PCR检测mec A基因,多重PCR进行SCC mec基因分型,基质辅助激光解吸电离飞行时间质谱(MALDI-TOF MS)技术进行同源性分析。结果44株MRSA通过普通PCR检测均含有mec A基因,多重PCR共分出Ⅱ、Ⅲ、Ⅳa 3种SCC mec基因型别,Ⅱ型5株(11.36%),Ⅲ型12株(27.27%),Ⅳa型8株(18.18%),未分型19株(43.18%)。MALDI-TOF MS将44株MRSA分为两大簇。结论SSTI患者分离的MRSA以SCCmecⅢ型为主,通过SCCmec基因分型和MALDI-TOF MS技术结合,为不同菌株间的同源性分析提供依据,并了解特定地区MRSA的分子生物学和流行病学特征。Objective To analyze methicillin-resistant Staphylococcus aureus(MRSA)isolated from patients with skin and soft tissue infection(SSTI),and to analyze the homology of staphylococcal cassette chromosome mec(SCC mec),so as to provide laboratory basis for standardizing the rational use of antibiotics and nosocomial infection control.Methods 44 strains of MRSA isolated from patients with SSTI were tested for antimicrobial susceptibility by K-B method and Vitek 2 Compact instrument.MecA gene was detected by ordinary PCR,SCC mec genotyping was performed by multiplex PCR,and homology was analyzed by matrix assisted laser desorption/ionization time of flight mass spectrometry(MALDI-TOF MS).Results Mec A gene was detected in all 44 MRSA strains by common PCR detection.Three SCC mec genotypes ofⅡ,ⅢandⅣa were identified by multiple PCR,including 5 strains of typeⅡ(11.36%),12 strains of typeⅢ(27.27%),8 strains of typeⅣa(18.18%)and 19 strains of untyped type(43.18%).44 MRSA strains were divided into two clusters by MALDI-TOF MS.Conclusion The main type of MRSA isolated from SSTI patients is SCCmecⅢtype.Through the combination of SCCmec genotyping and MALDI-TOF MS technology,it provides a basis for homology analysis among different strains and understands the molecular biology and epidemiological characteristics of MRSA in specific areas.
