川金丝猴肝硬化并发多脏器感染病例  

Liver Cirrhosis with Multi-Organism Infection of a Golden Snub-Nosed Monkey(Rhinopithecus roxellana)

摘　　要：一亚成体川金丝猴(Rhinopithecus roxellana)病例,临床主要表现为腹围增大、精神沉郁和运动无力;影像学检查提示胃、肠道有大量内容物和腹腔积液;血液检查血清蛋白偏低,天门冬氨转移酶偏高,白细胞升高;病理组织学检查见肺、胰腺、胃肠道及脾脏均呈现炎性病变,肝脏脂肪变性、纤维化、胆色素沉着;从腹腔积液中培养到1株专性厌氧的革兰氏染色阳性球菌,在普通血营养琼脂培养基中呈β溶血,与该菌株相似度较高的为链球菌(Streptococcus sp.I-P16),全基因相似度为95.92%;从肝组织培养到1株革兰氏阴性好氧杆菌,经16S rRNA基因比对,与弗格森埃希菌(Escherichia fergusonii)和大肠杆菌(E.coli)的染色体全基因相似度均为99.89%,经Biolog自动微生物鉴定系统鉴定为大肠杆菌。诊断为肝硬化并发多脏器细菌感染。In a case of a sub-adult golden snub-nosed monkey(Rhinopithecus roxellana),the clinical manifestations were mainly abdominal circumference enlargement,mental depression and motor weakness.The imaging examination showed that there were a large number of contents in the stomach and intestine,and effusion in the abdominal cavity.Blood analysis showed hypoproteinemia,elevated aspartate amino-transferase and white blood cells.Histopathological examination showed hepatic steatosis,fibrosis,biliary pigmentation,hemorrhagic pneumonia,pancreatitis,inflammatory lesions of the stomach,intestine,and spleen.One gram-stained positive coccus was cultured from peritoneal effusion,which showed-hemolysis in common blood nutrient agar medium.In addition,there was 95.92%gene sequences similar with Streptococcus sp.I-P16.A gram-negative aerobic bacilli was cultured from liver,and 99.89%of the total chromosome gene sequences was similar with that of both Escherichia fergusonii and E.coli,by using 16S rRNA sequence analysis.The same result,showing E.coli,was identified by Biolog automatic microbial identification system.In conclusion,cirrhosis complicated with multiple organ inflammatory lesions and bacterial infection was diagnosed.
