摘　　要：目的探讨自体脂肪颗粒注射移植联合A型肉毒毒素注射治疗重度眉间纹的临床效果。方法采用回顾性队列研究方法,选择2017年1月至2019年12月治疗的40例重度眉间纹患者。观察组20例,先进行A型肉毒毒素注射,1个月后行自体脂肪移植。对照组20例,仅行自体脂肪颗粒移植。术后1周、1个月、3个月、6个月和1年随访。随访时拍照,观察脂肪成活情况及并发症发生情况,并进行疗效评定。结果术后两组病例均存在移植脂肪吸收现象。观察组的疗效及患者满意率均显著高于对照组,差异具有统计学意义(P<0.05)。两组均未出现严重不良反应。结论自体脂肪颗粒注射移植联合A型肉毒毒素注射治疗重度眉间纹的临床效果优于单纯自体脂肪移植治疗;注射A型肉毒毒素1个月时,对于眉间肌肉效果作用最强,可以减少眉间肌肉运动并提高自体脂肪移植成活率,此时结合自体脂肪移植治疗可取得较好的临床疗效,值得临床推广使用。Objective To evaluate the clinical application of local autogenous fat grafting combined with botulinum toxin type A injection for glabellar wrinkles.Methods The retrospective cohort study was conducted.From January 2017 to December 2019,the clinical data of 40 cases with glabellar wrinkles were selected.In the observation group,20 cases were treated with autogenous fat grafting and botulinum toxin type A injection 1 month later.In the control group,only autologous fat grafting was performed in 20 patients.The patients were followed up at 1 week,1 month,3 months,6 months,and 1 year after operation.Photos were taken to observe the fat survival and complications.At the same time,the outcomes were evaluated.Results The phenomenon of fat absorption existed in both groups after fat grafted.The curative effect and satisfaction rate of patients in the observation group were significantly higher than those in the control group,and the difference between the two groups was statistically significant.There were no serious adverse reactions in the two groups.Conclusion Local autogenous fat grafting combined with botulinum toxin injection in the treatment of severe glabella wrinkles is better than that of autologous fat grafting alone.One month after botulinum toxin injection,the effect of botulinum toxin on the glabella muscle is the strongest,which can reduce the movement of the glabella muscle and improve the survival rate of fat transplantation.At this time,the combination with autologous fat transplantation can achieve good clinical efficacy,which is worthy of clinical promotion.
