摘  要:[目的]总结经皮内镜椎间孔入路椎管减压术治疗腰椎管狭窄症的临床效果。[方法]2016年1月-2019年12月,对43例椎管侧隐窝狭窄患者行经皮内镜椎间孔入路椎管减压术。总结患者的临床与影像资料。[结果]43例患者均顺利完成手术,无血管神经损伤等严重并发症。随时间推移,患者的VAS和0DI评分显著减少(P<0.05),而J0A评分显著增加(P<0.05)。随访过程中,43例患者均无疼痛或神经症状加剧,均无翻修手术。影像方面,责任节段椎管面积由术前(97.14±16.82)mm2,显著增加至末次随访时的(340.59±69.69)mm2,差异有统计学意义(P<0.05)。[结论]经皮椎间孔入路内镜下椎管减压术对椎管侧隐窝或神经根管狭窄的减压效果显著,临床效果满意。[Objective]To summarize the clinical outcomes of percutaneous endoscopic transforaminal decompression in the treatment of lumbar spinal stenosis.[Methods]From January 2016 to December 2019,43 patients with spinal canal stenosis were treated with percutaneous endoscopic transforaminal decompression.The clinical and imaging documents were summed up.[Results]All the 43 patients had operation completed successfully without serious complications such as vascular and nerve injury.The VAS and ODI scores decreased significantly(P<0.05),while JOA scores increased significantly over time(P<0.05).During the follow-up,none of the 43 patients had pain or neurological symptoms worsened,and none of them had revision surgery.In terms of imaging evaluation,the area of the spinal canal at the responsible segment increased significantly from(97.14±16.82)mm2 before surgery to(340.59±69.69)mm2 at the latest follow-up,which was statistically significant(P<0.05).[Conclusion]The percutaneous endoscopic transforaminal decompression does achieve sound clinical outcomes for lumbar spinal stenosis,especially the lateral recess stenosis or nerve root canal stenosis.
