宫颈分泌物胎儿纤维连接蛋白和血清松弛素表达与孕妇宫颈成熟度的相关性分析

摘　　要：目的观察孕妇宫颈分泌物胎儿纤维连接蛋白(fetal fibronectin, FFN)、血清松弛素(relaxin, RLX)的表达情况,探讨其与孕妇宫颈成熟度的相关性。方法 446例孕妇根据孕周分为孕24~27^(+6)组83例,孕28~36^(+6)组86例,孕37~38^(+6)组136例,孕39~41^(+6)组141例,采用ELISA法检测孕妇宫颈分泌物FFN和血清RLX水平,并进行4组间比较。根据临产情况将孕37~38^(+6)组和孕39~41^(+6)组孕妇再分为孕37~38^(+6)未临产组75例,孕37~38^(+6)自然临产组61例,孕39~41^(+6)未临产组62例,孕39~41^(+6)自然临产组79例。采用宫颈Bishop评分评估未临产组与自然临产组孕妇宫颈成熟度,比较宫颈分泌物FFN、血清RLX水平、宫颈Bishop评分;采用一般线性双变量Pearson直线相关性分析孕37~41^(+6)周孕妇宫颈分泌物FFN和血清RLX水平与宫颈成熟度(宫颈Bishop评分)的相关性。结果孕39~41^(+6)组孕妇宫颈分泌物FFN[(22.92±5.17)μg/L]和血清RLX水平[(401.59±36.52)ng/L]均高于孕24~27^(+6)组[(18.14±3.05)μg/L、(365.61±36.15)ng/L]、孕28~36^(+6)组[(17.96±3.14)μg/L、(362.81±35.67)ng/L]、孕37~38^(+6)组[(20.52±5.48)μg/L、(383.86±25.71)ng/L](P<0.05),孕37~38^(+6)组高于孕24~27^(+6)组、孕28~36^(+6)组(P<0.05),孕24~27^(+6)组与孕28~36^(+6)组比较差异无统计学意义(P>0.05);孕37~38^(+6)自然临产组和孕39~41^(+6)自然临产组孕妇宫颈分泌物FFN、血清RLX水平及宫颈Bishop评分均高于孕39~41^(+6)未临产组和孕37~38^(+6)未临产组(P<0.05),孕39~41^(+6)未临产组高于孕37~38^(+6)未临产组(P<0.05),孕37~38^(+6)组自然临产组与孕39~41^(+6)组自然临产组比较差异无统计学意义(P>0.05)。孕37~41^(+6)周孕妇宫颈分泌物FFN和血清RLX水平与宫颈成熟度均呈正相关(r=0.371,P<0.001;r=0.419,P<0.001)。结论孕37周及以后,孕妇宫颈分泌物FFN和血清RLX水平随孕周增长而逐渐增高,在未临产与自然临产的孕妇中,随宫颈分泌物FFN和血清RLX水平升高,宫颈成熟度逐渐升Objective To observe the expressions of fetal fibronectin(FFN) and serum relaxin(RLX) in cervical secretions, and to explore their correlations with maternal cervical maturity. Methods Totally 446 pregnant women were divided into 83 cases with gestational age of 24 to 27^(+6) weeks(gestational 24-27^(+6)-week group), 86 cases with gestational age of 28 to 36^(+6) weeks(gestational 28-36^(+6)-week group), 136 cases with gestational age of 37 to 38^(+6) weeks(gestational 37-38^(+6)-week group) and 141 cases with gestational age of 39 to 41^(+6) weeks(gestational 39-41^(+6)-week group). The levels of FFN in cervical secretion and RLX in serum were detected with ELISA and compared among four groups. Gestational 37-38^(+6)-week group was redivided into gestational 37-38^(+6)-week non-labor group(n=75) and gestational 37-38^(+6)-week natural labor group(n=61), and gestational 39-41^(+6)-week group was redivided into gestational 39-41^(+6)-week non-labor group(n=62) and gestational 39-41^(+6)-week natural labor group(n=79). Cervical Bishop score was used to evaluate the cervical maturity in non-labor groups and natural labor groups, and the levels of FFN in cervical secretion, RLX in serum and cervical maturity score were compared. The general linear bivariate Pearson linear correlation was used to analyze the correlations of cervical secretion FFN and serum RLX levels with maternal cervical maturity in pregnant women with gestational age of 37-41^(+6) weeks. Results The levels of FFN in cervical secretion and serum RLX level were higher in gestational 39-41^(+6)-week group [(22.92±5.17) μg/L,(401.59±36.52) ng/L] than those in gestational 24-27^(+6)-week group [(18.14±3.05) μg/L,(365.61±36.15) ng/L], gestational 28-36^(+6)-week group [(17.96±3.14) μg/L,(362.81±35.67) ng/L] and gestational 37-38^(+6)-week group [(20.52±5.48) μg/L,(383.86±25.71) ng/L](P<0.05), higher in gestational 37-38^(+6)-week group than those in gestational 24-27^(+6)-week group and gestational 28-36^(+6)-week group(P<0.05), and showed no
