摘 要:目的了解安徽省某三级甲等传染病医院医院感染现患率现状、经血传播病原体检出情况、医院感染危险因素和抗菌药物使用情况,为制定有效的感染防控措施提供循证依据。方法根据安徽省医院感染质控中心的统一要求,采用床旁调查和病例调查相结合的方式,对2016-2020年安徽省某三级甲等传染病医院所有住院患者的医院感染现状、经血传播病原体检出情况、医院感染危险因素和抗菌药物使用情况进行调查,并对数据进行分析。结果 2016-2020年医院感染现患率在1.31%~3.46%,感染科室均以重症监护病房和外科系统为主,感染部位以下呼吸道感染为主,医院感染病原体均以革兰氏阴性菌(G-菌)为主;经血传播病原体共检出1 065例次,占总数20.84%;抗菌药物使用率在46.19%~51.93%之间,抗菌药物送检率在42.48%~65.73%之间。结论 2016-2020年医院感染现患率均相对稳定,重症监护病房和下呼吸道是该院的重点科室和重点部位,经血传播病原体检出、职业防护、G-菌和临床抗菌药物使用等是该院院感防护的重点环节,应切实做好相关监测,落实院感防护措施,减少医院感染的发生。Objective To investigate the prevalence rate of nosocomial infection,the detection of blood-borne pathogens,the risk factors of nosocomial infection and the use of antibiotics in a grade-A tertiary infectious disease hospital in Anhui Province,provide evidence-based basis for formulating effective infection prevention and control measures. Methods According to the unified requirements of Anhui provincial hospital infection quality control center,the status of nosocomial infection,the de tection of blood-borne pathogens,the risk factors of nosocomial infection and the use of antibiotics in all hospitalized patients in a grade-A tertiary infectious disease hospital in Anhui Province from 2016-2020 were investigated by means of bedside investigation and case investigation. And the data were analyzed. Results From 2016 to 2020,the prevalence rate of nosocomial infection was from1.31% to 3.46%. The infection departments were mainly intensive care unit and surgical system,and the infection site was lower respiratory tract infection,and the pathogens of nosocomial infection were mainly Gram negative bacteria(G-bacteria). 1 065 cases of blood-borne pathogens were detected,accounting for 20.84% of the total. The utilization rate of antibacterial drugs was 46.19%-51.93%,and the detection rate of antibacterial drug was 42.48%-65.73%. Conclusion The prevalence rate of nosocomial infection from 2016-2020 is relatively stable. Intensive care unit and lower respiratory tract is the key departments and key parts respec-tively,and the detection of blood-borne pathogens,occupational protection,G-bacteria and clinical use of antibiotics are key protection links of nosocomial infection. It is necessary to improve the relevant monitoring and implement the hospital infection pro-tection measures,to reduce the nosocomial infection.
