摘 要:目的研究电针对骶髓损伤后神经源性膀胱尿潴留大鼠尿动力学的影响及作用机制。方法选择30只健康雄性大鼠,按照随机数字表法分为模型组、对照组、电针组,每组10只,对照组不建模、不治疗,模型组仅建模、不治疗,电针组建模后接受电针治疗,治疗后检测三组大鼠尿动力学变化及其他相关指标水平。结果模型组、电针组顺应性、漏尿点压力、膀胱基础压、膀胱最大容量均高于对照组,差异有统计学意义(P<0.05);电针组上述指标均低于模型组,差异有统计学意义(P<0.05)。模型组、电针组大鼠脊髓组织热休克蛋白20(HSP-20)、磷酸酶及张力蛋白同源物(PTEN)的mRNA水平均高于对照组,差异有统计学意义(P<0.05);电针组大鼠脊髓组织HSP20的mRNA水平高于模型组,而PTEN的mRNA水平低于模型组,差异有统计学意义(P<0.05)。模型组、电针组B淋巴细胞瘤-2基因(Bcl-2)、蛋白激酶B(Akt)水平均低于对照组,差异有统计学意义(P<0.05),模型组、电针组BCL2-Associated X蛋白质(Bax)、叉头框蛋白O1(FOXO1)、半胱氨酸天门冬氨酸蛋白酶-9(Caspase-9)均高于对照组,差异有统计学意义(P<0.05);电针组Bax、FOXO1、Caspase-9水平均低于模型组,Bcl-2高于模型组,差异有统计学意义(P<0.05),电针组、模型组Akt比较,差异无统计学意义(P>0.05)。结论电针治疗骶髓损伤后神经源性膀胱尿潴留大鼠能改善其尿动力学,具体作用机制分析与抑制PTEN表达、促进HSP20磷酸化与表达,并进一步抑制凋亡通路、提高Akt活性具有相关性。Objective To study the effect and mechanism of electroacupuncture on urodynamics of neurogenic bladder retention after sacral pulp injury in rats.Methods Thirty healthy male rats were selected and divided into model group,control group and electroacupuncture group according to random number table method,with 10 rats in each group.The control group was not modeled or treated,and the model group was only modeled and not treated,and the electroacupuncture group received electroacupuncture treatment after modeling.After treatment,the urodynamic changes and other related index in the three groups of rats were detected.Results Compliance,leakage point pressure,basic bladder pressure and maximum bladder volume in model group and electroacupuncture group were higher than those in control group,and the differences were statistically significant(P<0.05).The above indexes in electroacupuncture group were lower than those in model group,and the differences were statistically significant(P<0.05).The mRNA levels of heat shock protein 20(HSP-20),phosphatase and tensin homologous protein(PTEN)in spinal cord tissue of rats in model group and electroacupuncture group were higher than those in control group,and the differences were statistically significant(P<0.05).The mRNA level of HSP20 in electroacupuncture group was higher than that in model group,while the mRNA level of PTEN was lower than that in model group,with statistical significance(P<0.05).The levels of B-lymphocytoma-2 gene(Bcl-2)and protein kinase B(Akt)in model group and electroacupuncture group were lower than those in control group,the differences were statistically significant(P<0.05).BCL2-associated X protein(Bax),FOXO1 and caspase-9 in model group and electroacupuncture group were higher than those in control group,the differences were statistically significant(P<0.05).The levels of Bax,FOXO1 and Caspase-9 in electroacupuncture group were lower than those in model group,while the levels of Bcl-2 were higher than those in model group,the differences were statisti
