住院老年慢性阻塞性肺疾病患者衰弱状况及影响因素分析

摘　　要：目的观察住院老年慢性阻塞性肺疾病(COPD)患者衰弱发生情况,分析患者衰弱发生的影响因素。方法回顾性分析2018年2月至2020年2月收治168例住院老年COPD患者的资料,统计患者衰弱发生情况并分组,比较2组患者主要资料,将可能因素纳入,经单因素与多因素分析找出衰弱发生的影响因素。结果168例COPD患者,发生衰弱21例,发生率为12.50%;发生与未发生组的血红蛋白、疾病接受度、白介素-6(IL-6)、25-羟基维生素D(25-OH-D)比较差异有统计学意义(P<0.05),组间其他资料比较差异无统计学意义(P>0.05);Logistic回归分析结果显示,血红蛋白低表达、25-OH-D低表达和疾病接受度差均是住院老年COPD患者衰弱发生的危险因素(OR>1,P<0.05);IL-6低表达是住院老年COPD衰弱发生的保护因素(OR<1,P<0.05)。结论血红蛋白低表达、25-OH-D低表达、疾病接受度差和IL-6过表达均会增加住院老年COPD患者衰弱发生风险,临床应重视伴有上述风险因素的住院老年COPD患者的早期合理干预,以减少衰弱的发生。Objective To investigate the occurrence of weakness in hospitalized elderly patients with chronic obstructive pulmonary disease(COPD),and to explore its influencing factors.Methods The data of 168 elderly patients with COPD who were treated in our hospital from February 2018 to February 2020 were retrospectively analyzed.The patients were divided into weakness case group and non-weakness group.The data of patients were observed and compared between the two groups.Moreover the influencing factors on occurrence of weakness were investigated by using single factors and multi factor analysis.Results Of the 168 patients with COPD,21 cases had weakness,with the incidence rate being 12.50%.There were no significant differences in the levels of hemoglobin,disease acceptance degree,IL-6 and 25-OH-D as well as the other data between the two groups(P<0.05).Logistic regression analysis showed that low expression of hemoglobin,25-OH-D and poor disease acceptance were the risk factors of occurrence of weakness(P<0.05),however,the low expression of IL-6 was a protective factor of occurrence of weakness in patients with COPD(P<0.05).Conclusion The low expression of hemoglobin,25-OH-D,poor disease acceptance and overexpression of IL-6 may increase the risk of occurrence of weakness in hospitalized elderly patients with COPD,therefore,the clinicians should pay much attention to the early and reasonable intervention,so as to reduce the occurrence of weakness of the patients.
