芒针联合脊髓电刺激疗法治疗下肢缺血性疼痛及对患者疼痛与睡眠质量评分的影响

摘　　要：目的:探究芒针联合脊髓电刺激(SCS)疗法对下肢缺血性疼痛患者匹兹堡睡眠质量(PSQI)、视觉模拟评分(VAS)的影响。方法:选取下肢缺血性患者106例,按随机数字表法将患者分为SCS治疗组与联合治疗组各53例。其中SCS治疗组使用SCS疗法治疗,联合治疗组患者使用芒针联合SCS疗法治疗。红外热成像仪检测皮肤温差,多普勒血流控制仪检测踝肱指数(ABI),记录两组患者患肢冷感评分及麻木评分,记录患者间歇性跛行距离,经皮氧分压监测仪检测经皮氧分压(TcPO 2),VAS、PSQI评估患者疼痛状况与睡眠状况。结果:治疗后,与SCS治疗组相比,联合治疗组皮肤温差、踝肱指数均较高(P<0.05)。治疗后,与SCS治疗组相比,联合治疗组冷感评分、麻木评分均明显降低(P<0.05)。治疗后,与SCS治疗组相比,联合治疗组经皮氧分压、间歇性跛行距离均明显升高(P<0.05)。治疗后,与SCS治疗组相比,联合治疗组SAS评分、PSQI评分均较低(P<0.05)。治疗后,联合治疗组治疗有效率高于SCS治疗组(P<0.05)。结论:应用芒针联合SCS疗法对下肢缺血性疼痛患者进行治疗,能够有效改善患者病情,减轻冷感及麻木情况,改善皮肤组织的氧供量及下肢微循环,减轻患者疼痛,提高患者睡眠质量,效果明显。Objective:To investigate the effects of needle combined with spinal cord electrical stimulation(SCS)on pittsburgh sleep quality(PSQI)and visual analog scale(VAS)in patients with lower limb ischemic pain.Methods:A total of 106 patients with lower limb ischemia were selected and divided into SCS treatment group and combined treatment group(53 cases)according to random number table method.Patients in the SCS treatment group were treated with SCS therapy,and patients in the combined treatment group were treated with awn needle combined with SCS therapy.Skin temperature difference was detected by infrared thermal imaging,anklebral index(ABI)was detected by doppler flow control device,cold sensation score and numbness score of affected limbs of patients in both groups were recorded,intermittent claudicancy distance was recorded,transcutaneous partial oxygen pressure(TcPO 2)was measured by transcutaneous partial oxygen pressure monitor,pain status and sleep status of patients were evaluated by VAS and PQI.Results:After treatment,compared with SCS treatment group,skin temperature difference and ankle-brachial index were higher in combined treatment group(P<0.05).After treatment,compared with SCS treatment group,the score of cold sensation and numbness in the combined treatment group were significantly decreased(P<0.05).After treatment,compared with SCS treatment group,the percutaneous partial oxygen pressure and intermittent claudication distance in combined treatment group were significantly increased(P<0.05).After treatment,compared with SCS treatment group,SAS score and PSQI score of combined treatment group were lower(P<0.05).After treatment,the effective rate of combined treatment group was higher than that of SCS treatment group(P<0.05).Conclusion:The application of Awn needle combined with SCS therapy in the treatment of patients with lower limb ischemic pain can effectively improve the patient’s condition,reduce cold and numbness,improve the oxygen supply of skin tissue and lower limb microcirculation,alleviate
