脉冲染料激光联合强脉冲光治疗毛细血管畸形的疗效观察

摘　　要：目的探究脉冲染料激光(pulsed dye laser,PDL)联合强脉冲光(intense pulsed light,IPL)治疗毛细血管畸形的疗效。方法 2019年1—8月,将就诊于上海交通大学医学院附属第九人民医院激光美容科30例毛细血管畸形(capillary malformation,CM)患者病灶分为3部分,分别给予IPL+PDL治疗、PDL治疗,对照组区域不予治疗。治疗后3个月,通过视觉评估和光度计评价治疗效果并记录不良反应。结果治疗后3个月,IPL+PDL组、PDL组的红斑清除率分别为39%和34%(P>0.05)。IPL+PDL组与PDL组病灶颜色改善评分分别为(3.25±1.29)分和(3.22±1.31)分,两组VAS评分比较,其差异无统计学意义(P>0.05),两组均未出现明显的不良反应。结论 PDL联合IPL治疗CM是一种安全有效的治疗方法,但在本研究的治疗参数下,其与单独使用PDL相比,疗效未显著提高。Objective To investigate the efficacy of pulsed dye laser(PDL) combined with intense pulsed light(IPL) in the treatment of capillaary malformation(CM). Methods From January to August 2019, a total of 30 patients with CM were analyzed retrospectively. The lesions of CM were divided into IPL+PDL group, PDL group and control group. The IPL combined with PDL treatment was conducted in IPL+PDL group, the PDL treatment was performed in PDL group and no treatment was given to the control group. The efficacy was evaluated by visual assessment and photometer at 3 months after final treatment and adverse reactions were recorded. Results Compared with control group, there was significant improvement in IPL+PDL group and PDL group. The clearance rate of erythema was39% in IPL+PDL group and 34% in PDL group(P>0.05). The VAS scores were(3.25±1.29) in IPL+PDL group and(3.22±1.31) in PDL group(P>0.05). No obvious adverse reactions were reported. Conclusion The PDL combined with IPL is safe and effective in the treatment of CM. Under the treatment parameters in this study, no significant improvement in the efficacy is observed between IPL+PDL group and PDL group.
