针刺三阴交和曲池穴对原发性高血压疗效的Meta分析

摘　　要：目的:采用Meta分析方法评价针刺曲池和三阴交穴治疗高血压病的临床疗效。方法:检索CNKI中国期刊全文数据库、万方数据库、维普数据库、中国生物医学文献服务系、中国中医药数据库自建库以来到2020年6月所有针刺曲池和三阴交穴位治疗高血压病的临床随机对照试验(RCT)的文献,将临床总有效率和血压改善变化作为观察指标。采用Rev Man5.3软件对数据进行Meta分析。结果:两组临床总有效率比较,差异具有统计学意义,且观察组高于对照组[OR=4.28,95%的CI为(2.80~6.53),P<0.00001];两组舒张压比较,差异有统计学意义,且观察组好于对照组[MD=-11.19,95%的CI(-12.21~-10.17),P<0.00001];两组收缩压比较,差异有统计学意义,且观察组好于对照组[MD=-6.45,95%的CI(-7.06~-5.84),P<0.00001]。结论:针刺曲池、三阴交穴治疗原发性高血压具有较好的疗效,但现有的实验资料比较少,文献质量也比较低,大样本随机对照试验也比较缺乏,且针刺治疗也不宜使用盲法,以后需要更规范、更高质量大样本随机对照实验来验证疗效的真实可靠性。Objective:To evaluate the clinical efficacy of acupuncture at Quchi and Sanyinjiao points in the treatment of hypertension by Meta analysis.Methods:To search CNKI,Wanfang,Weipu,Chinese Biomedical Literature Service Department,and Chinese Traditional Chinese Medicine Database,all the literatures of clinical RCT experiments of acupuncture Quchi and Sanyinjiao acupoints in the treatment of hypertension since the establishment of the database to June 2020 were searched,and the clinical total effective rate and the improvement of blood pressure were taken as the observation indexes.Rev Man5.3 software was used for Meta-analysis of the data.Results:Comparing the clinical total effective rate between the two groups,the difference is statistically significant,and the observation group is better than the control group[OR=4.28,95%CI(2.80~6.53),P<0.00001].The difference in diastolic blood pressure between the two groups was statistically significant,and the observation group was better than the control group[MD=-11.19,95%CI(-12.21~-10.17),P<0.00001].Compared the two systolic blood pressures,the difference was statistically significant,and the observation group was better than the control group[MD=-6.45,95%CI(-7.06~-5.84),P<0.00001].Conclusion:Acupuncture at Quchi and Sanyinjiao points has a good curative effect in the treatment of essential hypertension,but the existing experimental data is relatively small,the quality of the literature is relatively low,large-sample randomized controlled trials are relatively lacking,and acupuncture is not easy to use Blind method,in the future,a more standardized,higher-quality large sample randomized controlled experiment is needed to verify the true reliability of the efficacy.
