摘 要:目的:探讨深度水解蛋白质配方奶在极低出生体质量儿早期喂养中的效用价值及安全性。方法:选择新生儿科监护室病房(NICU)2019年1月-2020年12月极低出生体质量儿108例,随机分为观察组(深度水解配方奶喂养)和对照组(标准早产儿配方奶喂养),每组54例。对比两组早产儿喂养耐受性、体格生长发育情况、肠外营养需要时间、达到全肠道喂养时间、恢复出生体质量时间。结果:观察组早产儿体质量、身长、头围增加速度明显快于对照组,差异有统计学意义(P<0.05);观察组需要肠外营养时间、达到全肠道营养时间和恢复出生体质量时间明显短于对照组,差异有统计学意义(P<0.05);观察组喂养不耐受如腹泻、腹胀以及新生儿坏死性小肠结肠炎发生率明显低于对照组,差异有统计学意义(P<0.05)。结论:极低出生体质量儿早期给予深度水解配方奶喂养,可促进其体格生长发育,缩短肠外营养应用时间,促进体质量增长,且喂养不耐受率远远低于标准早产儿配方奶。Objective:To investigate the efficacy and safety of deep hydrolyzed protein formula compared with standard premature formula in early feeding of very low birth weight infants.Methods:All 108 cases of very low birth weight infants in neonatal intensive care unit(NICU)of our hospital from January 2019 to December 2020 were randomly divided into observation group(54 cases)and control group(54 cases).The feeding tolerance,physical growth and development,the time required for parenteral nutrition,the time to reach total intestinal feeding,and the time to restore birth weight were compared between the two groups of preterm infants.Results:The weight,length,and head circumference of premature infants in the observation group increased significantly faster than those in the control group,and the difference was statistically significant(P<0.05).The weight,length,and head circumference of premature infants in the observation group increased significantly faster than those in the control group,and the difference was statistically significant(P<0.05).The incidence of feeding intolerance such as diarrhea,abdominal distension and neonatal necrotizing enterocolitis in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Early feeding of very low birth weight infants with deep hydrolysis formula can promote their physical growth and development,shorten the application time of parenteral nutrition,promote weight growth,and the feeding intolerance rate is far lower than that of standard preterm infant formula.
