活血止痛方联合膝关节清理术对气滞血瘀型轻中度膝骨性关节炎膝关节功能的影响

摘　　要：目的研究活血止痛方联合膝关节清理术对气滞血瘀型轻中度膝骨性关节炎患者膝关节功能的影响。方法选取2018年4月—2019年4月膝骨性关节炎患者114例,采用SPSS 20.0生成随机数字表,随机分为观察组和对照组,各57例。对照组采用膝关节清理术,观察组采用活血止痛方联合膝关节清理术;观察和比较两组患者治疗前后中医证候评分、西安大略麦马斯特大学骨性关节炎指数量表(WOMAC)评分和血清肿瘤坏死因子-α(TNF-α)、白介素-1β(IL-1β)水平及随访1年复发率。结果观察组治疗总有效率96.49%(55/57)高于对照组的84.21%(48/57),复发率1.75%(1/57)低于对照组的8.77%(5/57),差异有统计学意义(P<0.05)。治疗后较治疗前,两组患者中医主证、次证评分和膝关节疼痛、僵硬、关节功能评分、总分及TNF-α、IL-1水平均下降,且治疗后观察组低于对照组,差异均有统计学意义(P<0.05)。结论活血止痛方联合膝关节清理术方案相得益彰,进一步改善了膝关节功能,值得临床推广使用。Objective To study the effect of Huoxue Zhitong Recipe(活血止痛方) combined with knee joint debridement on the function of knee joint in patients with mild to moderate knee osteoarthritis of Qi stagnation and blood stasis type. Methods A total of 114 patients with knee osteoarthritis were selected from April 2018 to April 2019. A random number table was generated using SPSS 20.0 and the patients were randomly divided into an observation group and a control group, 57 cases in each. The control group was treated with knee joint debridement, and the observation group was treated with Huoxue Zhitong Recipe combined with knee joint debridement. The TCM syndrome scores, WOMAC score, serum levels of TNF-α, interleukin-1(IL-1) and relapse rate were observed and compared before and after treatment between the two groups. Results The total effective rate in the observation group was 96.49%(55/57), which was higher than 84.21%(48/57) in the control group, and the recurrence rate was 1.75%(1/57), which was lower than 8.77%(5/57) in the control group. It was statistically significant(P<0.05). After treatment, the scores of main syndrome, secondary syndrome, knee joint pain, stiffness, joint function, total score and levels of TNF-α and IL-1 in the two groups were decreased, and the observation group’s were lower than those of the control group and the differences were statistically significant(P<0.05). Conclusion The combination of Huoxue Zhitong Recipe and knee joint debridement program complements each other, which further improves the function of the knee joint and is worthy of clinical promotion.
