摘　　要：目的:探讨经椎间孔椎体间融合术(transforaminal lumbar interbody fusion,TLIF)附加单侧椎弓根钉内固定(unilateral pedicle screw fixation,uPS)治疗腰椎间盘突出症的中长期临床疗效。方法:对2014年7月-2016年12月采用单侧TLIF联合单侧椎弓根钉内固定治疗腰椎间盘突出症26例患者的临床资料及了年以上临床随访结果进行回顾性分析。结果:本组共26例患者.均获随访,随访时间38-62个月,平均40.6个月;26例患者均获得骨性椎间融合;按腰椎J0A评分标准评价:术前(8.21±2.12)分,末次随访(25.78±1.43)分,差异有统计学意义(t=4.95,P<0.05);J0A评分改善率82.13%±7.37%,优良率96.36%。结论:单侧TLIF联合单侧椎弓根钉内固定治疗腰椎间盘突出症,具有创伤小、并发症少、术后可早期负重锻炼及融合率高等优点,具有良好的临床疗效。Objective:To investigate the long-term clinical results of unilateral transforaminal lumbar interbody fusion with unilateral pedicle screw fixation for the treatment of lumbar disc herniation.Methods:From July 2013 to December 2015,26 patients with lumbar disc herniation were treated by the unilateral transforaminal lumbar interbody fusion with unilateral pedicle screw fixation were involved in our retrospective investigation and analyze the clinical follow-up results for not less than three years.Results:All patients were followed up prospectively for an average of 40.6 months(range 38 months to 62 months).26 cases got bony fusion at the last follow-up.According to the JOA criteria,there was significant difference(t=4.95,P<0.05)in JO A score between preoperation(8.21±2.12)and the last follow-up(25.78±1.43).In last follow-up,the rate of improvement was(82.13±7.37)%,with 96.36% of the total effec-tive rate.Conclusion:Using unilateral transforaminal lumbar interbody fusion with unilateral pedicle screw fixation for the treatment of lumbar disc herniation can led to a good functional outcome,because of the advantages,such as,minimally invasive,less complications,permission to early postoperative functional exercise,high fusion rate.
