基于经筋理论挑治配合芪虫通络汤治疗膝关节骨性关节炎的临床研究

摘　　要：目的观察基于经筋理论挑治配合芪虫通络汤治疗膝关节骨性关节炎(KOA)的临床疗效。方法选取2020年1月至2021年1月在东莞市桥头医院就诊的KOA患者70例为研究对象,采用随机数字表法分为试验组和对照组,每组各35例。对照组予口服塞来昔布胶囊、盐酸氨基葡萄糖胶囊和抗骨增生胶囊,试验组口服中药芪虫通络汤并配合病侧膝关节挑刺治疗,比较两组患者治疗前、治疗两个疗程后的骨关节炎调查量表(WOMAC)评分、疼痛视觉模拟(VAS)评分及临床疗效。结果与治疗前相比,治疗后两组患者日常生活难度、晨僵、疼痛评分均显著降低,而试验组降低程度更显著(P<0.05);治疗后两组患者VAS评分显著降低,试验组降低程度更显著(P<0.05);治疗后,试验组临床疗效总有效率为91.43%,显著高于对照组的82.86%,差异有统计学意义(P<0.05)。结论基于经筋理论挑治配合芪虫通络汤治疗KOA,能显著改善患者膝关节炎症状,降低疼痛程度,临床疗效显著,值得推广。Objective To observe the clinical efficacy on the pricking method based on the meridian sinew theory combined with Qichong Tongluo Decoction in the treatment of knee osteoarthritis(KOA).Methods A total of 70 patients with KOA in Qiaotou Hospital of Dongguan from January 2020 to January 2021 were selected as the study objects and randomly divided into the experimental group and the control group,with 35 cases in each group.The control group was treated with celecoxib capsule,glucosamine hydrochloride capsules and anti-hyperosteogeny capsule,while the experimental group was treated with Qichong Tongluo Decoction combined with pricking treatment of the affected knee joint.The osteoarthritis survey scale(WOMAC)scores,pain visual simulation(VAS)scores and clinical efficacy before treatment and after two courses of treatment were compared between the two groups.Results The scores of daily life difficulty,morning stiffness and pain of the two groups after treatment were significantly reduced than those before treatment,and the reduction of the experimental group was more significant(P<0.05).The VAS scores of patients in the two groups after treatment were significantly reduced,and the reduction of the experimental group was more significant(P<0.05).After treatment,the total clinical effective rate in the experimental group was 91.43%,which was significantly higher than that of 82.86%in the control group,with statistically significant difference(P<0.05).Conclusion The pricking method based on the meridian sinew theory combined with Qichong Tongluo Decoction in the treatment of KOA can significantly improve the symptoms of knee arthritis and reduce the degree of pain in patients with significant clinical efficacy,which is worthy of promotion.
