三甲复脉汤加味辨治原发性高血压眩晕(阴虚风动证)的效果及部分机制初探

摘　　要：目的观察三甲复脉汤加味辨治原发性高血压眩晕(阴虚风动证)的效果。方法选取2020年1月至2021年1月于济南市中医医院就诊的120例原发性高血压眩晕患者随机分为对照组(n=60)和观察组(n=60)。对照组给予常规西药治疗,观察组在对照组基础上给予三甲复脉汤加味方治疗,连续治疗4周。比较两组的血压控制、眩晕改善情况,监测血清中血栓前状态相关因子水平。结果治疗后,观察组收缩压(SBP)、舒张压(DBP)均显著低于对照组(P<0.05);治疗2周、治疗4周同一时间点两组比较,观察组欧洲眩晕评价量表(EEV)评分较对照组更低(P<0.05);观察组患者血清中血栓前状态相关指标血清凝血酶原激活物抑制剂-1(PAI-1)、血管性血友病因子(vWF)含量显著低于对照组(P<0.05)。结论三甲复脉汤加味方更利于控制患者的血压,减轻眩晕症状,其起效可能与改善血栓前状态有关,值得进一步推广。Objective To observe the effect of Sanjia Fumai Decoction with addition in the identification and treatment of primary hypertensive vertigo(yin deficiency and wind movement syndrome).Methods A total of 120 patients with primary hypertensive vertigo who attended Ji'nan Municipal Hospital of Traditional Chinese Medicine from Jan uary 2020 to January 2021 were randomly divided into the control group(n=60)and the observation group(n=60).The control group was given conventional western medicine treatment,and the observation group was given Sanjia Fumai Decoction with addition on the basis of the same treatment as the control group.Patients in both groups were treated for 4 weeks.The blood pressure control and improvement of vertigo were compared between the two groups,and levels of factors related to prethrombotic state in serum were monitored.Results After treatment,the systolic blood pressure(SBP)and diastolic blood pressure(DBP)in the observation group were significantly lower than those in the control group(P<0.05).The European evaluation of vertigo(EEV)scores in the observation group were significantly lower than those in the control group at the same time point of 2 weeks and 4 weeks after treatment(P<0.05).The prothrombin activator inhibitor-1(PAI-1)and von Willebrand Factor(vWF)levels of indicators related to prethrombotic state of the patients in the observation group were significantly lower than those in the control group(P<0.05).Conclusion Sanjia Fumai Decoction with addition is more beneficial to control patient's blood pressure and reduce vertigo,whose effect may be related to the improvement of prethrombotic state.Therefore,it is worthy of further promotion.
