摘 要:目的比较G-series系列培养基(Vitrolife)与Quinns advantage^(TM)序贯培养基(SAGE)对辅助生殖技术中胚胎体外发育和妊娠结局的影响。方法回顾性分析2015年1月至2020年9月于郑州大学第二附属医院生殖医学中心接受体外受精/卵胞浆内单精子注射-胚胎移植(in vitro fertilization/intracytoplasmic sperm injection-embryo transfer,IVF/ICSI-ET)患者的临床资料,按照年龄/体质量指数/获卵数/降调方案等筛选纳入人群,根据培养基及受精方式不同进行分组:采用IVF周期的分为Quinn’s组(289例)和Vitrolife组(99例),采用ICSI周期的分为Quinn’s组(161例)和Vitrolife组(60例)。比较同一受精方式两种不同培养基2PN受精率、2PN卵裂率、D3优质胚胎率、囊胚形成率、总优质胚胎率、临床妊娠率、胚胎植入率、早期流产率等的差异。结果不论IVF或ICSI周期,Vitrolife组的总优质胚胎率(IVF:60.62%vs 55.19%;ICSI:57.07%vs49.13%)、囊胚形成率(IVF:46.15%vs 34.93%;ICSI:49.17%vs 32.85%)、临床妊娠率(IVF:68.69%vs55.36%;ICSI:68.33%vs 49.69%)均显著高于Quinn’s组,差异均有统计学意义(P<0.05)。Logistics回归显示,不同培养基和移植胚胎数是影响临床妊娠的因素。结论与Quinn’s advantage^(TM)序贯培养基相比,使用G-series系列培养基进行体外培养,得到的优质胚胎更多,临床妊娠率也明显提高。Objective To compare the effects of G-series medium(Vitrolife)and Quinn’s advantage^(TM) sequential medium(SAGE)on human embryo development and pregnancy outcome in assisted reproductive technology.Methods Retrospective analysis of the clinical data of patients who underwent in vitro fertilization(IVF)or intracytoplasmic sperm injection(ICSI)from January 2015 to September 2020 in the Department of Reproductive Medicine of the Second Affiliated Hospital of Zhengzhou University.According to age/BMI/number of eggs obtained/down-regulation plan,the patients were included.According to the different culture medium and fertilization methods,they were divided into Quinn’s group(n=289)and Vitrolife group(n=99)in IVF cycle;Quinn’s group(n=161)and Vitrolife group(n=60)in ICSI cycle.In the same fertilization method,the 2 PN fertilization rate,2 PN cleavage rate,the high-quality embryo rate of the 3 days after fertilization,blastocyst formation rate,total high-quality embryo rate,clinical pregnancy rate,embryo implantation rate,early abortion rate were compared in the Vitrolife group and Quinn’s group.Results Regardless of the IVF or ICSI cycle,in the Vitrolife group,the total high-quality embryo rate(IVF:60.62%vs 55.19%;ICSI:57.07%vs 49.13%),blastocyst formation rate(IVF:46.15%vs 34.93%;ICSI:49.17%vs 32.85%),clinical pregnancy rate(IVF:68.69%vs 55.36%;ICSI:68.33%vs 49.69%)were significant higher than those in the Quinn’s group,with statistically significant differences(P<0.05).According to Logistics regression,different media and the number of embryos transfered were the influencing factors of clinical pregnancy.Conclusion Compared with Quinn’s advantage^(TM) sequential medium,G-series medium seems to be able to improve embryonic development ability and clinical pregnancy rate.
