新型挂线造口术对前庭大腺囊(脓)肿的疗效分析

摘　　要：目的探讨新型T管挂线造口术治疗前庭大腺囊(脓)肿的疗效。方法将2013年2月至2018年1月中国人民解放军中部战区武汉总医院妇科门诊部141例前庭大腺囊(脓)肿患者随机分为3组。A组50例行"T型挂线造口术",B组39例行"O型挂线造口术",C组52例行"传统造口术"。比较3组手术时间、术中出血量、术后换药次数、切口粘连率、复发率,探讨不同的造口术对治疗前庭大腺囊(脓)肿的临床可行性及优劣势。结果3组相比,A组手术时间[(307±51)s]、术中出血量[(1.3±0.5)m L]、平均换药次数[(4.2±0.5)次]、术后粘连率(2%)明显优于C组[(417±55)s、(2.8±0.8)m L、(7.3±1.3)次、82.7%],差异均有统计学意义(P<0.05)。A组复发率(0%)显著低于B、C组(5.1%、3.8%)。结论不同的造口方式治疗前庭大腺囊(脓)肿的疗效各有差异,其中T型挂线造口术局部反应小,效率确切,复发率低,值得临床推荐。Objective To explore the curative effect of new T-tube thread-hanging ostomy in the treatment of bartholin cyst(abscess).Methods From February 2013 to January 2018,141 patients with bartholin cyst(abscess)in General Hospital of Central Theater Command,Chinese People’s Liberation Army were randomly divided into 3 groups.Group A(50 cases)received"T-shaped string-hanging ostomy",group B(39 cases)received"O-shaped string-hanging ostomy",and group C(52 cases)had"traditional ostomy".The operation time,intraoperative blood loss,postoperative dressing change,incision adhesion rate,and recurrence rate were compared among the three groups,and the clinical feasibility,advantages and disadvantages of different ostomy treatments for the treatment of bartholin cyst(abscess)were discussed.Results Compared among the three groups,the operation time of group A[(307±51)s],intraoperative blood loss[(1.3±0.5)mL],average number of dressing changes[(4.2±0.5)times],postoperative adhesion rate(2%)were significantly better than group C[(417±55)s、(2.8±0.8)mL、(7.3±1.3)times、82.7%],the differences were statistically significant(P<0.05).The recurrence rate of group A(0)was significantly better than that of group B and C(5.1%,3.8%).Conclusion Different ostomy methods have different curative effects in the treatment of bartholin cyst(abscess).Among them,T-type thread-hanging ostomy has small local reaction,accurate efficiency,and low recurrence rate.It is worthy of clinical recommendation.
