不同剂量阿芬太尼复合丙泊酚用于无痛胃镜检查的临床观察

摘　　要：目的比较不同剂量阿芬太尼复合丙泊酚用于无痛胃镜检查的临床效果。方法选择宜昌市中心人民医院2020年6月至2021年5月行无痛胃镜患者128例为研究对象,采用随机数字表法分为四组,每组各32例。每组阿芬太尼的应用剂量为A1组(3µg/kg)、A2组(5µg/kg)、A3组(7µg/kg)、A4组(9µg/kg),比较四组患者检查时间、睁眼时间、清醒时间、清醒时视觉模拟评分(VAS)、丙泊酚总剂量及不良反应情况。结果四组患者检查时间、睁眼时间、清醒时间、清醒时VAS评分、眩晕、恶心、呕吐发生率比较,差异无统计学意义(P>0.05)。A4组丙泊酚总剂量明显少于A1、A2组,A3组丙泊酚总剂量明显少于A1组(P<0.05)。A4组体动发生率明显低于A1、A2组(P<0.05)。A1组呼吸抑制发生率明显低于A2、A3、A4组,A2、A3组呼吸抑制发生率明显低于A4组(P<0.05)。结论阿芬太尼7µg/kg复合丙泊酚用于胃镜检查中麻醉效果良好,呼吸抑制较轻,更为安全有效。Objective To compare the clinical effects of different doses of alfentanil combined with propofol administered for painless gastroscopy.Methods A total of 128 patients who underwent painless gastroscopy in the Central People's Hospital of Yichang from June 2020 to May 2021 were selected as the research objects and divided into 4 groups by the random number table method,with 32 cases in each group.The dosage of alfentanil in each group was A1(3µg/kg),A2(5µg/kg),A3(7µg/kg)and A4(9µg/kg).The examination duration,time to eye opening,awakening time,visual analogue scale(VAS)at the time of awakening,total dosage of propofol and adverse reactions were compared among the patients in the 4 groups.Results There were no statistically significant differences in the examination duration,time to eye opening,awakening time,VAS score at the time of awakening as well as the incidence of dizziness,nausea and vomiting among the four groups of patients(P>0.05).The total dosage of propofol in group A4 was significantly less than that in groups A1 or A2,and the total dosage of propofol in group A3 was significantly less than that in group A1(P<0.05).The incidence of body movement in group A4 was significantly lower than that in groups A1 or A2(P<0.05).The incidence of respiratory depression in group A1 w as significantly lower than that of the groups A2,A3,o r A4,a nd the incidence of respiratory depression in groups A2 or A3 was significantly lower than that of group A4(P<0.05).Conclusion Alfentanil(7µg/kg)combined with propofol has a good anesthesia effect in gastroscopy,with less respiratory depression,which is safer and more effective.
