眼睑部乳头状汗管囊腺癌1例

摘　　要：患者男,63岁。眼睑部肿物1年余,增大破溃3个月。左眼睑部单发鲜红色类圆形斑块,大小约1.5 cm×2.0 cm,部分覆有黑色痂皮,表面湿润、破溃,质稍硬。可见血性液体渗出。皮损组织病理示:真皮及皮下组织中可见呈巢片浸润性生长的肿瘤成分,细胞异型性明显,可见腺管及乳头状结构。免疫组织化学:p53(+),BCL-2(+),CK5/6(局灶+),p40(局灶+),p63(局灶+),CK7(局灶+),EMA(局灶+),AB-PAS(局灶+),CEA(-),S-100(-),SOX10(-),SyN(-),CgA(-),CD56(-),Ki-67(50%+)。结合以上结果,诊断为乳头状汗管囊腺癌。A case of syringocystadenocarcinoma papilliferum on the eyelid is reported.A 63-year-old man presented with a mass on his left eyelid for more than one year.The lesions became enlarged and ulcerated for three months.Physical examination revealed a single,round-like red plaque on the left cheek measuring 1.5 cm×2.0 cm,with moist and ulcerative surface partial covered by black scab.There was oozing of serosanguineous fluid and bleeding from the lesion.Histopathological examination showed infiltrative tumor components with significant cellular atypia in the dermis and subcutaneous tissue.Variable lumen formation and papillary structure was noted.Immunohistochemical analysis was positive for p53 and BCL-2.Part was positive for CK5/6,p40,p63,CK7,EMA and AB-PAS.The lesions were negative for CEA,S-100,SOX10,SyN,CgA and CD56.The positive rate of Ki-67 was about 50%.The diagnosis of syringocystadenocarcinoma papilliferum was made.
