64排容积CT双血供灌注参数在术前肺癌纵隔淋巴结转移诊断中的应用

摘　　要：目的:探讨64排容积CT双血供灌注参数在术前肺癌纵隔淋巴结转移诊断中的应用。方法:选取某院在2018年5月~2020年4月期间收治的肺癌患者82例作为本次研究对象,依据纵隔淋巴结是否转移,将其分为转移组(n=46)和非转移组(n=36)。采用飞利浦64排容积CT螺旋扫描仪对其进行检查,确诊灌注扫描范围后进行灌注扫描,获取15个扫描序列,将获取的数据传至CT后台进行图像处理,比较转移组和非转移组的肺动脉血流量(PAF)、灌注指数(PI)及支气管动脉血流量(BAF),记录BAF对术前纵隔淋巴结转移的诊断效能,并记录PI对术前纵隔淋巴结转移的诊断效能。结果:两组患者PAF差异无统计学意义(P>0.05);转移组PI显著低于无转移组,BAF显著高于无转移组(P<0.05);BAF的灵敏度为84.23%,特异度为83.62%,准确率为87.80%;PI的灵敏度为83.12%,特异度为82.35%,准确率为95.12%。结论:64排容积CT双血供灌注参数对术前肺癌患者的纵膈淋巴结转移有较高的诊断效能,可作为肺癌患者术前的评估依据。Objective:To explore the application of 64-slice volume CT dual blood perfusion parameters in the diagnosis of mediastinal lymph node metastasis of lung cancer before operation.Methods:A total of 82 lung cancer patients admitted to a hospital from May 2018 to April 2020 were selected as the subjects of this study.According to whether the mediastinal lymph nodes had metastasized,they were divided into metastatic group(n=46)and non-metastatic group(n=36).It was checked with a Philips 64-slice volume CT spiral scanner.After the perfusion scan range was confirmed,the perfusion scan was performed to obtain 15 scan sequences.The acquired data was sent to the CT background for image processing.The pulmonary arterial blood flow(PAF),perfusion index(PI),and bronchial arterial blood flow(BAF)of the metastatic group and the non-metastatic group were compared;The diagnostic efficacy of BAF and PI for preoperative mediastinal lymph node metastasis were recorded.Results:There was no statistically significant difference in PAF between the two groups of patients(P>0.05),the PI of the metastatic group was significantly lower than that of the non-metastatic group,and the BAF was significantly higher than that of the non-metastatic group(P<0.05).The sensitivity of BAF was 84.23%,the specificity was 83.62%,and the accuracy was 87.80%.The sensitivity of PI was 83.12%,the specificity was 82.35%,and the accuracy was 95.12%.Conclusion:The 64-slice volume CT dual blood supply perfusion parameters have high diagnostic efficiency for preoperative lung cancer patients with mediastinal lymph node metastasis,and can be used as a basis for preoperative evaluation of lung cancer patients.
