摘　　要：目的:利用格兰杰因果分析(Granger causality analysis,GCA)模型探究青少年肌阵挛癫痫(Juvenile Myoclonic Epilepsy,JME)患者有效连接。方法:采集20例青少年肌阵挛癫痫患者和20例健康被试的静息态功能磁共振成像(Resting-state functional MRI,rs-fMRI)数据。首先利用基于体素的形态学(voxel-based morphometry,VBM)方法分析两组被试灰质体积差异,获取右侧额中回、左侧海马旁回和左侧楔前叶等灰质异常脑区。然后以这3个脑区为感兴趣区通过格兰杰因果分析模型进行青少年肌阵挛癫痫有效连接研究。结果:青少年肌阵挛癫痫患者组的六条格兰杰因果关系中,右侧额中回到左侧海马旁回、右侧额中回到左侧楔前叶两条格兰杰因果关系均呈现负激活。结论:右侧额中回可能对青少年肌阵挛癫痫患者的脑功能网络异常有更大影响。Objective:This study explored the effective connection of Juvenile myoclonic epilepsy(JME)patients using Granger causality analysis(GCA).Methods:Resting state functional MRI(rs-fMRI)data were collected from 20 JME patients and 20 healthy subjects.The gray matter volume difference was analyzed between the two groups by using voxel based morphology(VBM)and three abnormal brain regions that included the right middle frontal gyrus,left parahippocampal gyrus and left precuneus were obtained.Then these three brain regions were taken as the regions of interest,and the effective connectivity in JME patients was studied by Granger causality analysis model.Results:Among the six Granger causalities in Juvenile myoclonic epilepsy group,the right middle frontal gyrus to the left parahippocampal gyrus and the right middle frontal gyrus to the left precuneus showed negative activation.Conclusion:The right middle frontal gyrus may have greater impact on brain functional network abnormalities in JME patients.
