多囊卵巢综合征患者血清Alarin水平与炎症状态及胰岛素抵抗的关系

摘　　要：目的探讨多囊卵巢综合征(polycystic ovary syndrome,PCOS)患者血清Alarin水平与炎症状态及胰岛素抵抗(insulin resistance,IR)的关系。方法选取2019年2月至2020年4月榆林市第一医院收治的80例初诊PCOS患者,根据BMI分为PCOS 1组(非肥胖型PCOS组,42例)和PCOS 2组(肥胖型PCOS组,38例),另选取同期30名健康受试者以及30名与PCOS 2组年龄、BMI相仿的单纯肥胖者设为对照1组、对照2组。测定所有受试者的人体学指标、激素水平、糖脂代谢指标、炎症因子及Alarin表达水平,对各指标进行Pearson相关性分析和多元逐步回归分析。结果Alarin的表达水平,对照2组[(0.32±0.06)ng/ml]、PCOS 1组[(0.35±0.06)ng/ml]和PCOS 2组[(0.48±0.11)ng/ml]均高于对照1组[(0.22±0.04)ng/ml,P<0.05],PCOS 2组高于对照2组和PCOS 1组(P<0.05)。Pearson相关性分析显示,Alarin与受试者的BMI、腰臀比、糖化血红蛋白(glycated hemoglobin,HbA1c)、空腹血糖(fasting blood glucose,FBG)、空腹胰岛素(fasting insulin,FINS)、胰岛素抵抗指数(Homa-IR)、雌二醇(estradiol,E_(2))、TG、超敏C反应蛋白(hypersensitive C-reactive protein,hs-CRP)及IL-6呈正相关(P<0.05),与HDL-C呈负相关(P<0.05)。多元线性回归分析显示,BMI(OR=2.408,95%CI:1.279~4.536,P=0.007)、Homa-IR(OR=5.355,95%CI:3.326~8.622,P=0.000)和IL-6(OR=1.855,95%CI:1.157~2.975,P=0.010)是Alarin的独立影响因素。结论Alarin在PCOS患者中呈现高表达,且与肥胖、IR及炎症反应密切相关;BMI、Homa-IR和IL-6是Alarin表达水平的独立影响因素。Objective To investigate the relation of serum Alarin with inflammatory status and insulin resistance(IR)in the patients with polycystic ovary syndrome(PCOS).Methods Eighty patients with PCOS admitted to Yulin First Hospital from February 2019 to April 2020 were divived into the PCOS 1 group(non-obese patients,42 cases)and PCOS 2 group(obese patients,38 cases).Thirty healthy subjects and 30 simple obese subjects were collected as control group 1,control group 2.The anthropometric indexes,hormone levels,glucose and lipid metabolism indexes,expression levels of inflammatory factors and Alarin were detected.The correlation analysis and multiple linear regression analysis were conducted.Results The expression levels of Alarin of control group 2[(0.32±0.06)ng/ml],PCOS 1 group[(0.35±0.06)ng/ml]and PCOS 2 group[(0.48±0.11)ng/ml]were higher than control group 1[(0.22±0.04)ng/ml,P<0.05],the Alarin of PCOS 2 group was higher than that of control group 2 and PCOS 1 group(P<0.05).The correlation analysis showed that the expression levels of Alarin was positively correlated with BMI,WHR,HbAlc,FBG,fasting insulin(FINS),Homa-IR,E_(2),TG,hs-CRP and IL-6(P<0.05),and negatively correlated with HDL-C(P<0.05).Multiple linear regression analysis showed that the expression levels of Alarin was independently associated with BMI(OR=2.408,95%CI:1.279-4.536,P=0.007),Homa-IR(OR=5.355,95%CI:3.326-8.622,P=0.000)and IL-6(OR=1.855,95%CI:1.157-2.975,P=0.010).Conclusions Alarin presents a high expression in the patients with PCOS,which is closely correlated with obesity,IR and inflammatory response.Alarin is independently associated with BMI,Homa-IR and IL-6.
