诺舒子宫内膜消融术与经宫腔镜子宫内膜电切术在异常子宫出血中的治疗效果与安全性

摘　　要：目的探讨诺舒子宫内膜消融术与经宫腔镜子宫内膜电切术在异常子宫出血(AUB)中的治疗效果与安全性。方法将广州市番禺区中心医院2010年3月-2017年2月收治的92例AUB患者随机分为经宫腔镜子宫内膜电切术治疗组(TCRE组)和诺舒子宫内膜消融术治疗组(NSEA组)各46例,对比两组手术中情况、治疗效果及并发症情况。结果NSEA组手术时间、术中出血量及住院时间明显少于TCRE组,差异有统计学意义(P<0.05)。术后1个月时TCRE组和NSEA组贫血患者的比例比术前显著降低,差异有统计学意义(P<0.05),术后6个月时两组均不再有贫血患者。术后6个月时NSEA组PBAC评分改善情况显著优于TCRE组,差异有统计学意义(P<0.05),而其他疗效指标变化差异无统计学意义(P>0.05)。两组术后并发症发生率差异无统计学意义(P>0.05)。结论诺舒子宫内膜消融术手术时间短,术中出血量少,明显减少了患者住院时间及PBAC评分,且术后并发症发生率低,值得在临床推广应用。Objective To explore the curative effect and safety of Nova Sure endometrial ablation and hysteroscopic endometrial resection in abnormal uterine bleeding(AUB).Methods From March 2010 to February 2017,92 patients with AUB treated in Central Hospital of Panyu District,Guangzhou were selected and randomly divided into hysteroscopic endometrial resection group(TCRE group)and Nova Sure endometrial ablation group(NSEA group),46 patients in each group.The intraoperative situations,curative effects,and complications in the two groups were compared.Results The operation time,the amount of intraoperative blood loss,and hospitalization time in NSEA group were statistically significantly less than those in TCRE group(P<0.05).At one month after operation,the proportions of patients with anemia in TCRE group and NSEA group were statistically significantly lower than those before operation(P<0.05);at six months after operation,no patient was found with anemia.At six months after operation,improvement of PBAC score in NSEA group was statistically significantly better than that in TCRE group(P<0.05),there was no statistically significant difference in the other indicators of curative effect(P>0.05).There was no statistically significant difference in the incidence rate of postoperative complications between the two groups(P>0.05).Conclusion Nova Sure endometrial ablation has a shorter operation time,less intraoperative blood loss,which significantly reduce hospitalization time and PBAC score,the incidence rate of postoperative complications is low,which is worthy of clinical promotion and application.
