摘 要:目的分析中性粒细胞CD64(CD64)、血清淀粉样蛋白A(SAA)及血清前白蛋白(PA检测在小儿呼吸道感染鉴别诊断中的应用价值。方法选取2018年1月至2020年5月本院收治的106呼吸道感染患儿作为研究组,根据实验室检查结果分为细菌感染组(n=54)和病毒感染组(n=52)。另选取98例同期于本院接受健康体检且结果正常的儿童作为健康对照组。比较各组CD64、SAA、PA水平,计算三者的阳性率,采用ROC曲线评估联合检测诊断小儿急性细菌性呼吸道感染的效能。结果研究组CD64、SAA水平高于对照组,PA水平低于对照组,差异均有统计学意义(P<0.05)。细菌感染组CD64、SAA水平高于病毒感染组,PA水平低于病毒感染组,差异均有统计学意义(P<0.05)。细菌感染组的CD64、SAA、PA阳性率(96.30%、100.00%、98.15%)高于病毒感染组(86.54%、92.31%、90.38%),差异均有统计学意义(P<0.05)。CD64、SAA、PA三者联合检测灵敏度、特异度分别为91.65、92.45,高于三者单一检测(P<0.05)。结论测定外周血中CD64、SAA、PA水平,可有效鉴别呼吸道感染类型。Objective To analyze the value of cluster of differentiation 64(CD64),serum amyloid A(SAA)and serum prealbumin(PA)detection in the differential diagnosis of respiratory tract infections in children.Methods The 106 children with respiratory tract infections admitted to this hospital from January 2018 to May 2020 were selected as the research group.According to the results of laboratory tests,the patients were divided into bacterial infection group(n=54)and virus infection group(n=52).In addition,98 children who had physical examination in our hospital during the same period and had normal results were selected as the healthy control group.The levels of CD64,SAA,and PA in each group were compared,the positive rates of them were calculated,and the ROC curve was used to evaluate the efficacy of combined detection in the diagnosis of acute bacterial respiratory infections in children.Results The CD64 and SAA levels in the study group were higher than those in the control group,and the PA level was lower than that in the control group,and the difference was statistically significant(P<0.05).The levels of CD64 and SAA in the bacterial infection group were higher than those in the virus infection group,and the PA level was lower than that in the virus infection group,the difference was statistically significant(P<0.05).The positive rates of CD64,SAA and PA in the bacterial infection group(96.30%,100.00%,98.15%)were higher than those in the viral infection group(86.54%,92.31%,90.38%),and the difference was statistically significant(P<0.05).The sensitivity and specificity of the combined detection of the three indicators were 91.65 and 92.45 respectively,which were significantly higher thanthose of the single detection of CD64,SAA,and PA(P<0.05).Conclusion Determination of the levels of CD64,SAA and PA in peripheral blood can effectively identify the types of respiratory tract infections.
