摘　　要：目的分析炔雌醇环丙孕酮片联合二甲双胍治疗多囊卵巢综合征(PCOS)的性激素结合球蛋白(SHBG)、胰高血糖素样肽-2(GLP-2)及酪酪肽(PYY)水平变化。方法选取本院2018年6月至2019年9月收治的106例PCOS患者作为研究组,其中Ⅰ型患者13例,Ⅱ型患者19例,Ⅲ型患者43例,Ⅳ型患者31例,另选取同期在本院门诊体检的120例健康人群作为健康对照组。比较不同人群、不同时期SHBG、GLP-2及PYY水平变化趋势,采用Pearson分析,绘制散点图,分析治疗前SHBG、GLP-2及PYY水平与患者病情程度的相关性。结果治疗前研究组SHBG、GLP-2及PYY水平均显著低于健康对照组,差异均有统计学意义(P<0.05)。治疗前不同亚型SHBG、GLP-2及PYY水平显示:Ⅳ型>Ⅲ型>Ⅱ型>Ⅰ型,差异有统计学意义(P<0.05)。Pearson相关性分析结果显示:治疗前SHBG、GLP-2及PYY水平与PCOS病程分型呈正相关(P<0.05)。PCOS患者治疗后SHBG、GLP-2及PYY水平明显高于治疗前,差异均有统计学意义(P<0.05)。结论炔雌醇环丙孕酮片联合二甲双胍治疗PCOS患者临床疗效显著,可提升患者SHBG、GLP-2及PYY水平,检测上述水平可为临床评估PCOS患者病情进展情况提供参考。Objective To analyze changes of sex hormone-binding globulin(SHBG),glucagon like peptide-2(GLP-2)and Peptide YY(PYY)levels in patients with polycystic ovary syndrome(PCOS)treated with ethinyl estradiol and cyproterone tablets combined with metformin.Methods A total of 106 patients with PCOS admitted to this hospital from June 2018 to September 2019 were selected as the study group,including 13 cases in typeⅠ,19 cases in typeⅡ,43 cases in typeⅢand 31 cases in typeⅣ.In addition,120 healthy people who had physical examination in the outpatient clinic of this hospital during the same period were selected as the healthy control group.The changes of SHBG,GLP-2 and PYY levels in different groups and different periods were compared.Pearson analysis and scatter plot were used to analyze the correlation between the levels of SHBG,GLP-2 and PYY before treatment and the patient.s condition.Results Before treatment,the levels of SHBG,GLP-2 and PYY in the study group were significantly lower than those in the healthy control group,and the differences were all statistically significant(P<0.05).The different subtypes and the severity ofthe disease before treatment showed that the levels of SHBG,GLP-2 and PYY increased significantly with the severity of the disease,and the differences were statistically significant(P<0.05).The Pearson correlation analysis showed that before treatment,the levels of SHBG,GLP-2 and PYY were positively correlated with the type of PCOS course(P<0.05).The levels of SHBG,GLP-2 and PYY after treatment in PCOS patients were significantly higher than those before treatment,and the differences were statistically significant(P<0.05).Conclusion Ethinyl estradiol and cyproterone tablets combined with metformin have a significant clinical effect in the treatment of patients with PCOS,which can increase the levels of SHBG,GLP-2 and PYY in patients.The detection of the three can provide a reference for clinical assessment of the progress of patients with PCOS.
