局部刺络放血治疗痤疮的前瞻性研究

摘　　要：目的观察局部刺络放血治疗痤疮的临床疗效和安全性。方法选取痤疮患者307例,随机分为观察组(154例)和对照组(153例)。对照组予常规药物治疗,观察组在对照组治疗基础上予皮损局部刺络放血治疗。比较两组临床疗效和不良反应发生情况,观察两组治疗前和治疗后1周、2周、1月的痤疮整体严重度评分(evaluator globe severity score, EGSS)的变化。结果两组临床疗效比较,轻度痤疮组间差异无统计学意义(P>0.05),中度痤疮组间差异具有统计学意义(P<0.05)。两组GAGS评分比较,轻度痤疮不同时间点组内比较差异具有统计学意义(P<0.05),两组组间比较差异无统计学意义(P>0.05);中度痤疮不同时间点组内比较差异有统计学意义(P<0.05),两组组间比较差异具有统计学差异(P<0.05)。结论在常规药物治疗基础上,局部刺络放血治疗能提高中度痤疮的临床疗效,促进中度痤疮的皮损消退。Objective To observe the clinical efficacy and safety of topical collateral-pricking and bloodletting in treating acne. Method A total of 307 patients with acne were enrolled and randomized into an observation group(154 cases) and a control group(153 cases). The control group was intervened by conventional medication and the observation group was given topical collateral-pricking and bloodletting at the skin lesions. Clinical efficacy and adverse reactions were compared between the two groups. Prior to treatment, and at 1 week, 2 weeks and 1 month of treatment, the evaluator’s global severity score(EGSS) was observed in both groups. Result There was no significant difference in clinical efficacy for mild acne cases between the two groups(P>0.05), while the between-group difference in clinical efficacy for moderate acne cases was statistically significant(P<0.05). Regarding the EGSS score in mild acne cases, the intra-group differences between different time points were statistically significant in both groups(P<0.05), while the between-group differences were statistically insignificant(P>0.05);in moderate acne cases, the intra-group differences in the EGSS score were statistically significant in both groups(P<0.05), and the between-group differences were also statistically significant(P<0.05). Conclusion Based on conventional medication, topical collateral-pricking and bloodletting can enhance clinical efficacy and promote the recovery of skin lesions for moderate acne.
