摘 要:目的探讨百草枯中毒肺损伤患者白细胞介素6(IL-6)、可溶性白细胞介素6受体(sIL-6R)、可溶性糖蛋白130(sgp130)水平变化及其临床意义。方法选取2017年12月至2019年12月重庆市巴南区人民医院收治的百草枯中毒合并肺损伤患者50例(观察组),选取同期健康体检者30例(健康对照组)。采用双抗体夹心酶联免疫吸附法检测两组血清IL-6、sIL-6R和sgp130水平,并进行比较。采用Pearson相关性分析对血清IL-6、sIL-6R和sgp130水平与百草枯中毒肺损伤患者急性生理与慢性健康状况评估-Ⅱ(APACHE-Ⅱ)评分的关系进行分析。采用受试者工作特征(ROC)曲线分析血清IL-6、sIL-6R及sgp130水平对百草枯中毒致肺损伤患者的诊断价值。结果观察组血清IL-6、sIL-6R、sgp130水平高于健康对照组[(108.62±11.39)ng/L比(57.41±7.63)ng/L、(73.28±6.94)ng/L比(45.13±6.57)ng/L、(435.64±36.52)mg/L比(281.71±68.35)mg/L],差异有统计学意义(P<0.05)。相关性分析结果显示,观察组血清IL-6、sIL-6R、sgp130水平与APACHE-Ⅱ评分呈正相关(r=0.824、0.937、1.215,P<0.05)。ROC曲线分析结果显示,当血清IL-6、sIL-6R、sgp130水平分别以最佳诊断临界值为83.96 ng/L、68.51 ng/L和367.42 mg/L诊断百草枯中毒致肺损伤时,其灵敏度分别为84.80%、78.20%、87.10%,特异度分别为70.30%、85.50%、81.00%。结论血清IL-6、sIL-6R及sgp130水平与百草枯中毒致肺损伤患者的病情严重程度密切相关,IL-6、sIL-6R及sgp130水平检测对百草枯中毒致肺损伤有较好的诊断价值。Objective To investigate the change of interleukin-6(IL-6),soluble interleukin-6 receptor(sIL-6R)and soluble glycoprotein 130(sgp130)levels in patients with lung injury caused by paraquat poisoning and its clinical significance.Methods Fifty patients with paraquat poisoning and lung injury admitted to Ba′nan District People′s Hospital of Chongqing from December 2017 to December 2019 were selected as the observation group,and 30 healthy patients were selected as the control group.The serum levels of IL-6,sIL-6R and sgp130 in two groups was detected with double-antibody sandwich enzyme-linked immunosorbent assay and compared.Pearson correlation analysis was performed on the relationship between serum IL-6,sIL-6R and sgp130 and the acute physiological and chronic health status assessment(APACHE)-Ⅱscore of patients with lung injury caused by paraquat poisoning.The receiver operating characteristics(ROC)curve was used to analyze the diagnostic value of serum IL-6,sIL-6R and sgp130 on paraquat poisoning-induced lung injury.Results The serum levels of IL-6,sIL-6R,and sgp130 in the observation group were significantly higher than those in the control group:(108.62±11.39)ng/L vs.(57.41±7.63)ng/L,(73.28±6.94)ng/L vs.(45.13±6.57)ng/L,(435.64±36.52)mg/L vs.(281.71±68.35)mg/L,and the differences were statistically significant(P<0.05).The results of correlation analysis showed that the serum levels of IL-6,sIL-6R,and sgp130 in the observation group were significantly positively correlated with the APACHE-Ⅱscore(r=0.824,0.937,1.215,P<0.05).ROC curve analysis results showed that when serum IL-6,sIL-6R and sgp130 had the best diagnostic cut-off values of 83.96 ng/L,68.51 ng/L and 367.42 mg/L respectively for the lung injury caused by paraquat poisoning,the sensitivity was 84.80%,78.20%and 87.10%,and the specificity was 70.30%,85.50%and 81.00%.Conclusions The expression levels of IL-6,sIL-6R and sgp130 in serum are closely related to the severity of lung injury caused by paraquat poisoning,and IL-6,sIL-6R and sgp130
