摘 要:目的:探讨不同心率对320排容积冠状动脉CT血管成像(CCTA)图像质量和辐射剂量的影响。方法:将60例临床怀疑冠心病(CHD)行320排容积CCTA检查的患者,根据扫描时心率分为2组,A组心率≤75次/min,采用1个心动周期扫描,采集时相为70%~80%R-R间期;B组心率>75次/min,采用2个心动周期(75~100次/min)或3个心动周期(>100次/min)扫描,采集时相为35%~55%R-R间期。对2组图像质量、辐射剂量进行统计学分析。结果:心率越低则图像质量评分越高,A组图像质量评分高于B组,差异有统计学意义(P<0.05),A组辐射剂量(ED)为(1.11±0.78)mSv,B组ED为(3.73±0.95)mSv,A组辐射剂量比B组降低约70%(P<0.05)。结论:行320排容积CCTA检查的患者,控制心率降至75次/min以下,能有效提高图像质量和减少辐射剂量,值得推广应用。Purpose:To investigate the effects of different heart rates on image quality and radiation dose of 320-row volume CT coronary angiography(CCTA).Methods:Sixty patients with clinically suspected coronary heart disease(CHD)were divided into two groups according to their heart rate at the time of scanning.Patient in group A(heart rate≤75 beats/min)was scanned by one cardiac cycle,and the acquisition phase was 70%-80%R-R interval.Patient in group B(heart rate>75 beats/min)was scanned in two or three cardiac cycles(75-100 beats/min)or three cardiac cycles(>100 beats/min),and the acquisition phase was 35%-55%R-R interval.The image quality and radiation dose of the two groups were analyzed.Results:The lower the heart rate,the higher the image quality score.The image quality score of group A was higher than that of group B,and the difference was with statistical significance.The radiation dose(ED)of group A was(1.11±0.78)mSv,and the ED of group B was(3.73±0.95)mSv.In group A,the radiation dose was about 70%lower than that in group B(P<0.05).Conclusions:When the heart rate is reduced to less than 75 beats/min in patients with 320-row volume CCTA,the image quality can be effectively improved and the radiation dose can be reduced,which is worth popularizing.
