一例犬气肿性膀胱炎的病例分析

摘　　要：1只7岁金毛巡回犬尿血达两个月,为了进行有效的诊治,对患犬进行了X射线和B型超声波等影像学检查,尿常规检查、尿液细菌分离和药敏试验以及血常规、血液生化指标等实验室检查。结果表明:X光片在膀胱区域可见气体密度影像,B型超声波检查结果提示膀胱壁内积气;尿沉渣镜检可见大量红细胞和杆状细菌,有的中心粒细胞有胞内吞噬细菌现象;尿液经细菌分离与鉴定得到大肠杆菌,该分离菌株对阿莫西林克拉维酸钾、头孢氨苄、恩诺沙星、多西环素等药物均敏感;血常规及血液生化指标检查结果除胆固醇、三酰甘油含量升高外,其余指标均无明显异常。综合全部检查结果诊断为大肠杆菌感染引发的气肿性膀胱炎。根据药敏试验结果进行抗菌治疗,首次治疗1个月后,患犬未及时复诊,两个月后复发,再次治疗1个月患犬转归良好。说明该方法对犬气肿性膀胱炎有明显的治疗效果。A 7-year-old golden retriever was admitted to the hopital with hematuria for two months. In order to carry out effective diagnosis and treatment, the sick dog received the imageological examinations including X-ray, B-ultrasound, urine routine examination, bacterial culture, antimicrobial susceptibility test, blood routine and blood biochemistry indexes. The results showed that X-ray showed gas density images in the bladder area, and B-mode ultrasound results indicated gas accumulation in the bladder wall;microscopic examination of urinary sediment showed a large number of red blood cells and rod-shaped bacteria, some centrioles had intracellular phagocytosis of bacteria and the bacteria was identified as Escherichia coli by bacterial isolation and identification from urine. The isolated strain was sensitive to drugs such as amoxicillin-clavulanate potassium, cephalexin, enrofloxacin, doxycycline. Except for the elevated levels of cholesterol and triacylglycerol, the blood routine and blood biochemical index examination results showed no obvious abnormalities in the other indexes. Based on all the examination results, the dog was diagnosed with emphysema cystitis caused by E. coli infection. According to the results of drug susceptibility test, the dog was treated with antimicrobial therapy;one month after the first treatment, the affected dog did not return in time, and the disease relapsed two months later, and after another month of treatment, the affected dog returned well. It showed that this method had obvious therapeutic effect on caine emphysema cystitis.
