归桃麻子仁丸加减治疗老年2型糖尿病便秘的临床观察

摘　　要：目的:观察经典方剂麻子仁丸加用化瘀通络药物当归、桃仁在老年糖尿病便秘患者中的疗效。方法:采用病例随机对照研究方法,随机选取2019-2020年潮州市中心医院老年2型糖尿病患者60例做为研究对象,分为治疗组和对照组各30例。通过观察指标糖化血红蛋白(Glycosylated Hemoglobin,HbA1C)变化、便秘症状评分前后变化进行疗效判定。结果:中药治疗组的临床总有效率和症状总积分的改善比西药对照组更具优势,两组之间的差异具有统计学意义;并且对排便周期、大便性状改善情况明显优于对照组。结论:治病必求本是中医学的治疗原则也是优势,同样在糖尿病便秘的治疗中,应从病因病机出发,审证求因,标本同治。血瘀机制在糖尿病便秘发病过程中的影响不容忽视,在润肠通便的基础上加用化瘀通络是可行的,为糖尿病便秘的中医临床治疗提供更加广阔的思路。Objective:To observe the efficacy of the classic prescription,Maziren Pills plus Angelica sinensis and peach kernel(Guitao)that are used for removing blood stasis and dredging collaterals,in elderly patients with type 2 diabetic constipation.Methods:Using a case control study method,60 elderly patients with type 2 diabetes in Chaozhou Central Hospital from 2019 to 2020 were randomly selected as the research objects;They were divided into two groups evenly,a treatment group and a control group.The curative effect was judged by observing the changes of index glycosylated hemoglobin(HbA1 C)and the constipation symptom score before and after treatment.Results:The total clinical effective rate and the improvement of the total symptom score of the treatment group are better than the control group that was using western medicines,and the difference between the groups is statistically significant.Additionally,for the treatment group,the improvement of defecation cycle and stool characteristics is significantly better than the control group.Conclusion:Treatment should focus on the principal cause of a disease.This is not only the treatment principle but also the advantage of our national medicine.Hence,for the treatment of Type-2 Diabetes Constipation,we should start from the etiology and pathogenesis,verify the basic cause among different symptoms,and then determine treatment with universal methods.Herein,the influence of blood stasis mechanism in the onset of type 2 diabetic constipation cannot be ignored.Our results show that adding blood stasis and dredging collaterals on the basis of moistening the intestines and laxative is feasible,which provides a broader way for the clinical treatment of type 2 diabetic constipation.
