电针配合穴位贴敷治疗气滞血瘀型老年膝骨关节炎的临床疗效及对患者膝关节功能、血液流变学和生活质量的影响

摘　　要：目的观察电针配合穴位贴敷治疗气滞血瘀型老年膝骨关节炎(KOA)的临床疗效及对患者膝关节功能、血液流变学和生活质量的影响。方法将134例老年KOA患者按照随机数字表法分为2组,对照组67例予电针治疗,治疗组67例在对照组治疗基础上配合穴位贴敷治疗。2组均隔日治疗1次,每周治疗3次为1个疗程,连续治疗3个疗程。比较2组疗效;观察2组治疗前后Lysholm膝关节功能评分表(LKS)评分、血液流变学指标、生活质量评分变化。结果治疗组总有效率95.52%(64/67),对照组总有效率82.09%(55/67),治疗组疗效优于对照组(P<0.05)。2组治疗后疼痛、不安定度、闭锁感及肿胀度、行动情况评分均较本组治疗前升高(P<0.05),且治疗组升高更明显(P<0.05)。2组治疗后全血黏度(中切)、血浆黏度、红细胞聚集指数均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗后生理功能、活力、情感、社交、总体健康评分均较本组治疗前升高(P<0.05),且治疗组升高更明显(P<0.05)。结论电针配合穴位贴敷治疗气滞血瘀型老年KOA,可改善患者膝关节功能,降低血液黏度,提高患者生活质量。Objective To observe the effects of electroacupuncture combined with acupoint application for knee joint function,hemorrheology and quality-of-life in senile knee osteoarthritis(KOA)of qi-stagnation and blood-stasis.Methods Totally 134 senile KOA were randomly divided into 2 groups,67 cases in control group were given electroacupuncture,and 67 cases in treatment group were additionally given acupoint application on the basis of control group.The treatment was given on alternate days,3 times for a course,continued for 3 cycles.The key observation was the changes of Lysholm knee score(LKS),hemorheology,and quality-of-lifescore;the curative effect was counted.Results The total effective rates in treatment group and control group were 95.52%(64/67)and 82.09%(55/67),respectively,the curative effect being better in treatment group than control group(P<0.05).Compared with before treatment,LKS(pain,instability,locking of knee,swelling degree,action situation)in groups remarkably increased(P<0.05),which increased notably in treatment group when compared with cortrol group(P<0.05);hemorheology(whole blood viscosity,plasma viscosity,red cell aggregation index)in groups were significantly downregulated(P<0.05),which downregulated notably in treatment group when compared with cortrol group(P<0.05);quality-of-lifescore(physiological function,vigor,emotion,social contact,overall health)in groups were higher after treatment(P<0.05),which in treatment group were significantly increased(P<0.05).Conclusion For senile KOA(qi-stagnation and blood-stasis),the combination of the above two can improve knee joint function,reduce blood viscosity,and improve the quality-of-life.
