摘  要:目的探讨鞘内注射甘珀酸(carbenoxolone,CBX)对大鼠趾部切口痛的影响及其作用机制。方法将SD大鼠随机分为对照组、模型组、生理盐水组、CBX组(0.6μg/10μL)、抑制剂组(1.25μg/10μL)。生理盐水组、CBX组、抑制剂组大鼠于术前30 min经鞘内分别注射生理盐水、CBX、CBX抑制剂10 panx,均10μL/只,模型组不给药;采用经典Brennan法对大鼠进行趾部切口痛模型操作,对照组不给药不进行手术处理。于术前30 min及术后2、4、6、24 h测定各组大鼠的机械缩足阈值(mechanical withdrawal threshold,MWT)和热缩足潜伏期(thermal withdrawal latency,TWL)。于术后2、4、6、24 h,Western blot法测定各组大鼠脊髓腰膨大中缝隙连接蛋白1(pannexin 1,PX1)的表达水平。于术后2 h,免疫荧光法检测对照组、模型组、CBX组、抑制剂组大鼠脊髓腰膨大的PX1及与脊髓星形胶质细胞活化标志物胶质纤维酸性蛋白(glial fibrillary acidic protein,GFAP)的共表达情况。结果各组大鼠术前MWT及TWL差异均无统计学意义(P> 0.05)。与对照组比较,模型组和生理盐水组术后2、4、6 h的MWT明显降低(P <0.001);各时间点的TWL明显降低(P <0.001),PX1的表达水平明显升高(P <0.05)。与模型组比较,生理盐水组各时间点MWT、TWL、PX1差异均无统计学意义(P> 0.05);CBX组及抑制剂组术后2、4 h的MWT均明显升高(P <0.001),2、4、6 h的TWL明显升高(P <0.001);CBX组2、4、24 h的PX1表达水平明显降低(P <0.05),抑制剂组2、4、6、24 h的PX1表达量明显降低(P <0.05)。与CBX组比较,抑制剂组各时间点的MWT差异无统计学意义(P>0.05),术后2和24 h的TWL明显升高(P <0.05);术后2、6 h的PX1蛋白表达水平明显降低(P <0.05),术后4 h明显升高(P <0.05)。模型组的PX1和GFAP共表达最明显,CBX组的共表达有所减少,抑制剂组减少最显著。结论 CBX可缓解大鼠趾部切口痛,可能是通过抑制PX1的表达及星形胶质细胞的活化而发挥作用的。Objective To investigate the effect of intrathecal injection with carbenoxolone(CBX)on toe incision pain in rats as well as the relevant mechanism.Methods SD rats were randomly divided into control,model,normal saline,CBX(0.6 μg/10 μL)and inhibitor(1.25 μg/10 μL)groups.Rat model of toe incision pain was established by typical Brennan method.The rats in normal saline,CBX and inhibitor groups were intrathecally injected with normal saline,CBX and 10 panx 30 min before operation respectively,10 μL for each,while those in model group were not injected,and those in control group were untreated.The mechanical withdrawal threshold(MWT) and foot withdrawal latency(TWL)were measured 30 min before and 2,4,6 and 24 h after operation.The expression levels of pannexin 1(PX1)in the lumbar segment of the spinal cord of rats 2,4,6 and 24 h after operation were determined Western blot,while the co-expression of PX1 and glial fibrillary acidic protein(GFAP) 2 h after operation were observed by IFA.Results No significant differences were observed in the MWTs and TWLs of rats in various groups before operation(P > 0.05).Compared with those in control group,the MWTs in model and normal saline groups 2,4 and 6 h after operation decreased significantly(P < 0.001),while the TWLs at various time points decreased significantly(P < 0.001),and the expression level of PX1 increased significantly(P < 0.05).Compared with those in model group,the MWTs,TWLs and expression levels of PX1 in normal saline group at various time points showed no significant difference(P > 0.05),while the MWTs in CBX and inhibitor groups 2 and 4 h after operation increased significantly(P < 0.001),the TWLs 2,4 and 6 h after operation increased significantly(P < 0.001),the expression levels of PX1 in CBX group 2,4 and 24 h and in inhibitor group 2,4,6 and 24 h after operation decreased significantly(each P < 0.05).However,compared with those in CBX group,the MWTs in inhibitor group at various time points showed no significant difference(P > 0.05),while the TWLs 2
