精囊镜在顽固性血精治疗中的应用

摘　　要：目的研究精囊镜在顽固性血精治疗中的应用效果。方法选取2018年1月至2020年1月东莞市人民医院及东莞市大朗医院采用精囊镜手术治疗的40例顽固性血精患者的临床资料纳入手术组,另选取2015年12月至2017年12月采用药物治疗的41例顽固性血精患者的临床资料纳入药物组。分析两组在临床疗效、治疗前后直肠指检精囊腺触痛程度、治疗前后精液红细胞计数和白细胞浓度、并发症发生情况等方面的差异。结果手术组治愈率高于药物组,复发率低于药物组,差异具有统计学意义(P<0.05);治疗后两组直肠指检精囊腺触痛视觉模拟评分法(VAS)评分均低于治疗前,且手术组低于药物组,差异具有统计学意义(P<0.05);治疗后手术组红细胞计数、白细胞浓度均低于药物组,差异具有统计学意义(P<0.05);两组尿道狭窄、急性附睾炎及发热发生率比较,差异无统计学意义(P>0.05)。结论精囊镜在顽固性血精治疗中的应用效果显著,有利于提高临床疗效,减轻直肠指检精囊腺触痛,同时减少精液红细胞计数、白细胞浓度,安全性较佳。Objective To study the application effect of seminal vesiculoscopy in patients with refractory hematospermia.Methods The clinical data of 40 patients with intractable hematospermia treated by seminal vesi-coscopy in Dongguan People′s Hospital and Dongguan Dalang Hospital from January 2018 to January 2020 were in-cluded in the surgery group,and 41 patients with intractable hematospermia treated by drug from December 2015 to December 2017 were included in the drug group.The differences between the two groups in clinical efficacy,the tenderness degree of seminal vesicle gland in digital rectal examination before and after treatment,the concentration of semen red and white blood cells before and after treatment,and the incidence of complications were analyzed.Results The cure rate of the operation group was higher than that of the drug group,and the recurrence rate was lower than that of the drug group,with statistical significance(P<0.05).After treatment,VAS scores of seminal vesicle tenderness in digital rectal examination of the two groups were lower than before,and the operation group was lower than the drug group,the difference was statistically significant(P<0.05);erythrocyte count and white blood cell concentration in the operation group were lower than those in the drug group(P<0.05).There were no signifi-cant differences in the incidence of urethral stricture,acute epididymitis and fever between the two groups(P>0.05).Conclusions The application of seminal vesiculoscopy in patients with refractory hematospermia has a sig-nificant effect,which is beneficial to improve the clinical efficacy,reduce the tenderness of rectal digital examina-tion of the seminal vesicle,and reduce the red blood cell count and leukocyte concentration,with good safety.
