孟鲁司特钠对中重度慢性阻塞性肺疾病急性加重期患者的疗效

摘　　要：目的探讨中重度慢性阻塞性肺疾病急性加重期(AECOPD)患者在常规治疗的基础上,应用孟鲁司特协同治疗后,对肺功能、生活质量、运动耐力及血气的影响。方法选取80例中重度AECOPD患者,随机分为对照组和观察组,每组40例。对照组患者进行抗生素、祛痰、平喘、糖皮质激素和氧疗等常规治疗,观察组在对照组治疗的基础上加用孟鲁司特钠,治疗前及治疗2周后分别记录2组患者肺通气功能指标、慢性阻塞性肺疾病评估测试(CAT)评分、6 min步行试验距离(6MWD)和血气分析结果,并进行比较。结果治疗后,2组患者肺通气功能指标、6MWD和氧合指数均明显高于治疗前,且观察组升高更显著;2组患者CAT评分、二氧化碳分压均明显低于治疗前,且观察组降低更显著,差异有统计学意义(P<0.05)。结论孟鲁司特钠可通过提高肺通气功能、改善健康状态、提高运动耐力、纠正呼吸衰竭等使中重度AECOPD患者获得一定疗效。Objective To explore the efficacy of montelukast sodium treatment in patients with acute exacerbation of moderate-to-severe chronic obstructive pulmonary disease(AECOPD)on the basis of routine treatment,and to study the effect on lung function,quality of life,exercise tolerance and blood gas.Methods Eighty patients with AECOPD were randomly divided into control group and observation group,with 40 cases in each group.The control group were treated with antibiotics,expectorant,anti-asthmatics,glucocorticoids,and oxygen therapy,while the observation group was given oral montelukast sodium on the basis of control group treatment.Before treatment and after 2 weeks of treatment,the lung function index,competency assessment tool(CAT)score,6 minutes walking distance(6MWD)and blood gas analysis results of the 2 groups were recorded and compared.Results After treatment,the indexes of lung ventilation function,6MWD and oxygenation index of the 2 groups were significantly higher than before treatment,and the observation group increased more significantly.The CAT score and carbon dioxide partial pressure of the 2 groups were significantly lower than before treatment,and the observation group decreased more significantly,the differences were statistically significant(P<0.05).Conclusion Montelukast sodium can achieve certain effects in patients with moderate to severe AECOPD by improving lung ventilation function,improving health status,improving exercise endurance,and correcting respiratory failure.
