抚触护理配合耳道护理对新生儿听力筛查测试结果的影响

摘　　要：目的观察抚触护理配合耳道护理对新生儿听力筛查测试结果的影响。方法选择2018年2月—2019年5月期间于莆田市第一医院出生的216例新生儿作为研究对象,将其随机分为联合组(n=108)与基础组(n=108)。联合组新生儿使用抚触护理联合耳道护理,基础组单独使用基础耳道护理。记录两组新生儿出生3 d及42 d后听力筛查测试通过情况(出生3 d后初筛通过率、出生42 d后复筛通过率)、测试过程中镇静状态优良率、两组新生儿听力测试过程中情绪由躁动变为安静的时间以及疼痛紧张程度[新生儿疼痛评估量表(neonatal infants pain scales,NIPS)评分]。结果出生3 d后两组初筛通过率比较,差异无统计学意义(P>0.05),出生42 d后,联合组复筛通过率明显高于基础组(P<0.05);出生3 d及42 d后,联合组新生儿测试过程中镇静状态优良率明显高于基础组(P<0.05);出生42 d后,联合组情绪由躁动变为安静的时间以及NIPS得分明显低于出生3 d后(P<0.05),且明显低于基础组(P<0.05);基础组出生42 d后情绪由躁动变为安静的时间、NIPS得分与出生3 d后比较,差异无统计学意义(P>0.05)。结论抚触护理配合耳道护理能够提高新生儿听力筛查通过率,安抚新生儿情绪。Objective To observe the effect of touch nursing combined with ear canal nursing on newborn hearing screening test results.Methods A total of 216 newborns born in Putian First Hospital from February 2018 to May 2019 were selected as the study subjects and randomly divided into combined group(n=108)and basic group(n=108).Neonates in the combined group were given touch care combined with ear canal care,while those in the basic group were given basic ear canal care alone.The pass rate of hearing screening test(pass rate of primary screening after 3 days of birth,pass rate of re-screening after 42 days of birth),the excellent rate of sedation during the test,the change of mood from restlessness to quiet time and the degree of pain tension during the hearing test of newborns in both groups were recorded(neonatal infants pain scales,NIPS).Results There was no significant difference in the pass rate of primary screening between the two groups after 3 days of birth(P>0.05).After 42 days of birth,the pass rate of re-screening in the combined group was significantly higher than that in the basic group(P<0.05).After 3 days and 42 days of birth,the excellent rate of sedation in the newborn test in the combined group was significantly higher than that in the basic group(P<0.05).After 42 days of birth,the mood changed from agitation to quiet time and NIPS in the combined group.The NIPS was significantly lower than that after 3 days of birth(P<0.05),and was significantly lower than that after 42 days of birth in the basic group(P<0.05).After 42 days of birth in the basic group,the mood changed from agitation to quiet time and NIPS of emotions from tremor to reality were compared with those after 3 days of birth,with no significant differences(P>0.05).Conclusion Touching nursing combined with ear canal nursing can improve the pass rate of newborn hearing screening and soothe newborn's mood.
