阿托伐他汀联合氯吡格雷治疗冠心病心绞痛的效果观察

摘　　要：目的分析阿托伐他汀联合氯吡格雷治疗冠心病心绞痛的效果。方法抽取2020年5月至2021年5月濮阳市安阳地区医院收治的冠心病心绞痛患者78例,按随机数字表法分为联合组(39例)与单一组(39例),单一组采用阿托伐他汀治疗,联合组在单一组基础上联合氯吡格雷治疗。对比两组疗效、心绞痛症状改善情况和治疗前后心电图参数、纤溶状态、血液流变学变化。结果联合组总有效率(97.44%)高于单一组(82.05%),P<0.05;治疗后,联合组心绞痛发作频次较单一组少,持续时间较单一组短(P<0.05);治疗后,联合组ST段压低总和(ΣST)、ST段下降导联数(NST)较单一组低(P<0.05);治疗后,联合组组织型纤溶酶原激活物抑制剂-1(tPAI-1)、可溶性E-选择素(sE-selectin)、可溶性细胞间黏附分子-1(sICAM-1)较单一组低(P<0.05);治疗后,联合组红细胞平均体积(MCV)、红细胞压积(HCT)较单一组低(P<0.05)。结论阿托伐他汀联合氯吡格雷治疗冠心病心绞痛能提高疗效,改善纤溶状态和血液流变学,改善心电图参数,缓解心绞痛症状。Objective To analyze the efficacy of atorvastatin combined with clopidogrel in the treatment of angina pectoris of coronary artery disease.Methods A total of 78 patients with angina pectoris of coronary artery disease in Anyang District Hospital of Puyang,from May 2020 to May 2021 were selected,and divided into combined group(39 cases)and single group(39 cases)according to the random number table method.The single group was treated with atorvastatin,and the combined group was treated with clopidogrel on the basis of the single group.The curative effect,the improvement of angina pectoris symptoms and the changes of ECG parameters,fibrinolytic status and hemorheology before and after treatment were compared between the two groups.Results The total effective rate of the combined group(97.44%)was higher than that of the single group(82.05%),P<0.05.After treatment,the frequency of angina pectoris in the combined group was less than that of the single group,and the duration was shorter than that of the single group(P<0.05).After treatment,the sum of ST segment depression(ΣST)and number of ST segment descending leads(NST)of the combined group were lower than those of the single group(P<0.05).After treatment,tissue plasminogen activator inhibitor-1(tpai-1),soluble E-selectin(sE-selectin)and soluble intercellular adhesion molecule-1(sICAM-1)in the combined group were lower than those in the single group(P<0.05).After treatment,mean corpuscular volume(MCV)and hematocrit(HCT)of the combined group were lower than those of the single group(P<0.05).Conclusions Atorvastatin combined with clopidogrel in the treatment of angina pectoris of coronary artery disease can improve the efficacy,improve the fibrinolytic state and hemorheology,improve the electrocardiogram parameters,and relieve the symptoms of angina pectoris.
