Effects of high protein diet on renal function,weight,and lipids in overweight/obese adults

摘  要:目的:研究高蛋白膳食模式对超重/肥胖成人肾功能、体重、血脂的影响,为后续研究高蛋白膳食对超重和肥胖的干预提供参考依据。方法:招募140名20~45岁超重/肥胖者,利用简单随机化法分为高蛋白膳食组(HP组:蛋白质供能比30%)和标准蛋白膳食(SP组:蛋白质供能比15%),干预60 d,观察两组体重、体脂肪、肾功能(肌酐、尿素氮、尿酸)及血脂(低密度脂蛋白、总胆固醇、三酰甘油)变化。结果:受试者140例,男性41例,女性99例,其中HP组70例,SP组70例。第一周,HP组能量和SP组比较差异无统计学意义(P>0.05);HP组蛋白质、碳水化合物、脂肪供能比与SP组比较差异有统计学意义(P<0.001);第五周,HP组能量与SP组比较差异无统计学意义(P>0.05);HP组蛋白质、碳水化合物、脂肪供能比与SP组比较差异有统计学意义(P<0.001)。干预前,HP组体重、体脂肪与SP组比较差异无统计学意义(P>0.05),干预后,两组指标均较干预前有所改变,差异有统计学意义(P<0.05),且HP组与SP组体重下降率比较差异有统计学意义(P<0.001);HP组与SP组体脂肪下降率比较差异有统计学意义(P<0.001),且HP组体重、体脂肪下降更明显。血脂变化情况:干预前,HP组甘油三酯、低密度脂蛋白、总胆固醇与SP组比较差异无统计学意义(P>0.05);干预后,两组指标均较干预前有所改变,差异有统计学意义(P<0.05),且HP组甘油三酯、低密度脂蛋白、总胆固醇下降率与SP组比较差异均有统计学意义(P<0.001),且HP组下降比SP组明显。肾功能变化:干预前,HP组肌酐、尿素氮、尿酸与SP组比较差异无统计学意义(P>0.05),干预后,与基线值相比,两组肌酐均有所下降,差异有统计学意义(P<0.05),SP组尿酸有所降低,差异有统计学意义(P<0.05),尿素氮差异无统计学意义(P>0.05),HP组尿酸差异无统计学意义(P>0.05),尿素氮有所降低,差异有统计学意义(P<0.05)。两组间比较显示HP组尿素氮、尿Objective:It amis to study the effects of high protein diet on overweight/obese adults in terms of renal function,body weight and blood lipid,therefore providing reference for the follow-up study on the intervention of high protein diet on overweight and obesity.Methods:A total of 140 overweight/obese subjects aged 20-45 were randomly divided into high-protein diet group(HP group:protein energy ratio was 30%)and a standard-protein diet(SP group:protein energy ratio was 15%)for 60 days.The body weight,body fat,renal function(creatinine,urea nitrogen,uric acid),blood lipids(low density lipoprotein,cholesterol,triglyceride)were observed before and after intervention.Results:140 subjects with HP(n=70)or SP(n=70)were enrolled,including 41males and 99 females.Compared with baseline value,the weight of HP group and SP group decreased by 8.20±2.42%and 3.50±2.17%,respectively(P<0.001);The body fat decreased by 21.72±7.32%and 8.32±5.19%,respectively(P<0.001);Tiglyceride,cholesterol,low density lipoprotein and creatinine all decreased(P<0.001);In SP group,uric acid was decreased(P<0.001)and urea nitrogen was not changed(P=0.463),while in HP group,uric acid was not changed(P=0.074)and urea nitrogen was decreased(P<0.001).Compared with SP group,the weight,body fat,triglyceride,cholesterol,low density lipoprotein and urea nitrogen in HP group decreased more significantly(P<0.001),but there was no difference in creatinine between the two groups(P=0.750).Conclusion:The effects of high protein diet was better than that of standard protein diet on reducing body weight,body fat,blood lipids and urea nitrogen.The level of uric acid decreased more significantly in standard protein diet than in high protein diet.High protein diet had no adverse effect on renal function.
