精蛋白锌重组赖脯胰岛素混合注射液(25R)联合二甲双胍治疗2型糖尿病患者的效果

摘　　要：目的探讨2型糖尿病患者给予精蛋白锌重组赖脯胰岛素混合注射液(25R)配以二甲双胍治疗的效果。方法选取2019年9月—2020年9月镇江市丹徒区人民医院收治的2型糖尿病患者108例,按照电脑产生随机数字表法分为对照组和观察组,各54例。对照组施以精蛋白锌重组赖脯胰岛素混合注射液(25R),观察组施以精蛋白锌重组赖脯胰岛素混合注射液(25R)联合二甲双胍治疗。比较2组患者血糖指标水平、血糖达标时间、胰岛素β细胞功能及不良反应发生率。结果治疗前,2组空腹血糖(FBG)、餐后2 h血糖(2 hPG)、22:00血糖、3:00血糖、糖化血红蛋白(HbA_(1c))比较,差异无统计学意义(P>0.05)。治疗后,观察组FBG、2 hPG、22:00血糖、3:00血糖、HbA1c低于对照组(P<0.05)。观察组PBG、早中晚餐前空腹血糖(PPBG)、早中晚餐后2 h血糖(2 hPG)达标时间短于对照组(P<0.05)。治疗前,2组空腹胰岛素(FIns)、稳态模型胰岛素抵抗指数(HOMA-IR)、胰岛素β细胞功能指数(HOMA-β)比较,差异无统计学意义(P>0.05)。治疗后,观察组FIns、HOMA-IR低于对照组,HOMA-β高于对照组(P<0.05)。2组患者不良反应总发生率比较,差异无统计学意义(P>0.05)。结论精蛋白锌重组赖脯胰岛素联合二甲双胍治疗2型糖尿病患者,可降低其血糖水平,缩短PBG、早中晚PPBG、早中晚2 hPG达标时间,提升胰岛素β细胞功能,且不会引起诸多不良反应。Objective To investigate the therapeutic effect of protamine zinc recombinant insulin lispro mixed injection(25R) combined with metformin in the treatment of type 2 diabetes mellitus.Methods 108 patients with type 2 diabetes mellitus in People’s Hospital of Dantu District from September 2019 to September 2020 as the research subjects.They were divided into control group and observation group by computer generated random number method,with 54 cases in each group.The control group was treated with protamine zinc recombinant insulin lispro mixed injection(25 R),and the observation group was treated with protamine zinc recombinant insulin lispro mixed injection(25R) combined with metformin respectively.The levels of blood glucose,the time of blood glucose reaching the standard,the function of insulin beta cells and the incidence of adverse reactions were compared between the two groups.Results Before treatment,there was no significant differences in FPG,2 h postprandial blood glucose(2 hPG),22:00 blood glucose,3:00 blood glucose and glycosylated hemoglobin(HbA_(1c)) between the two groups(P>0.05).After treatment,FBG,2 hPG,22:00 blood glucose,3:00 blood glucose and HbA1c in the observation group were lower than those in the control group(P<0.05).The standard time of PBG,fasting blood glucose(PPBG) before breakfast,lunch and dinner and 2 h blood glucose(2 hPG) after breakfast,lunch and dinner in the observation group was shorter than that in the control group(P<0.05).Before treatment,there was no significant differences in Fins,HOMA-IR,HOMA-β between the two groups(P>0.05).After treatment,Fins and HOMA-IR in the observation group were lower than those in the control group,HOMA-β was higher than that in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Protamine zinc recombinant insulin lispro mixed injection(25 R) combined with metformin can reduce blood glucose level in patients with type 2 diabetes,short the standard
