补阳还五汤加减治疗带状疱疹后遗神经痛的临床疗效

摘　　要：目的探讨补阳还五汤加减治疗带状疱疹后遗神经痛的临床疗效。方法选取宁德市中医院2018年12月—2020年12月收治的带状疱疹后遗神经痛患者50例,采用随机数字表法分为对照组与观察组,每组25例。对照组予以甲钴胺片联合普瑞巴林胶囊治疗,观察组在对照组基础上联合补阳还五汤加减治疗。比较2组临床疗效,治疗前后视觉模拟评分法(VAS)评分、阿森斯失眠量表(AIS)评分、皮肤病生活质量指数(DLQI)评分、β-内啡肽(β-EP)、P物质,并观察2组不良反应发生情况。结果观察组治疗总有效率高于对照组(P<0.05)。治疗前2组VAS评分、AIS评分、DLQI评分比较,差异无统计学意义(P>0.05);治疗后观察组VAS评分、AIS评分、DLQI评分低于对照组(P<0.05)。治疗前2组β-EP、P物质比较,差异无统计学意义(P>0.05);治疗后观察组β-EP高于对照组,P物质低于对照组(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论补阳还五汤加减治疗带状疱疹后遗神经痛的临床疗效确切,可有效减轻患者疼痛程度,改善睡眠质量,提高生活质量,且安全性较高。Objective To investigate the clinical effect of modified Buyanghuanwu decoction on postherpetic neuralgia.Methods A total of 50 cases of patients with postherpetic neuralgia were selected from December 2018 to December 2020 in Ningde Hospital of Traditional Chinese Medicine,which were randomly divided into control group and observation group,25 cases in each group.The control group was treated with mecobalamin tablets combined with pregabalin capsules,the observation group was treated with Buyanghuanwu decoction on the basis of the control group.Clinical efficacy,visual analogue scale(VAS)score,athens Insomnia Scale(AIS)score,dermatology quality of life index(DLQI)score,β-endorphin(β-EP)and substance P before and after treatment were compared between the two groups,and the incidence of adverse reactions were observed.Results The total effective rate of observation group was higher than that of control group(P<0.05).There was no significant difference in VAS score,AIS score and DLQI score between the two groups before treatment(P>0.05);after treatment,the VAS score,AIS score and DLQI score in observation group were lower than those in control group(P<0.05).Before treatment,there was no significant difference inβ-EP and P between the two groups(P>0.05);after treatment,β-EP in observation group was higher than that in control group,but Substance P was lower than that in control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Modified Buyanghuanwu decoction have an exact clinical effect for postherpetic neuralgia,which can effectively reduce the pain degree of patients,improve the sleep quality and improve the quality of life,and with high safety.
