探析血清PCT和CRP水平对预测小儿高热惊厥合并感染的临床价值

摘　　要：目的:探讨血清降钙素原(PCT)与C-反应蛋白(CRP)水平在预测小儿高热惊厥合并感染中的价值。方法:回顾性分析2017年1月~2020年1月102例小儿高热惊厥患儿的临床资料,将28例无感染者作为无感染组,将25例局部感染者作为局部感染组,将26例重症肺炎者作为重症肺炎组,将23例败血症者作为败血症组。四组患儿均进行血清PCT、CRP水平检测,分析其诊断感染及预测患儿预后的价值。结果:伴随感染程度的加重,患儿血清PCT、CRP水平明显提升,差异有统计学意义(P<0.05);血清PCT联合CRP水平检测评估患儿合并感染程度的敏感度、准确度和阴性预测值均高于单纯血清PCT、CRP检测,差异有统计学意义(P<0.05);血清PCT联合CRP水平检测评估患儿合并感染程度的敏感度、准确度和阴性预测值均高于单纯血清PCT、CRP检测,差异有统计学意义(P<0.05)。结论:血清PCT联合CRP水平检测可有效评估小儿高热惊厥患儿是否合并感染,并判断其感染严重程度,并预测其预后效果。Objective To explore the value of serum procalcitonin(PCT)and C-reactive protein(CRP)levels in the prediction of infection in children with febrile convulsions.Method A retrospective analysis of the clinical data of 102 children with febrile convulsions in our hospital from January 2017 to January 2020,28 patients without infection were regarded as no infection group,25 patients with local infection were regarded as local infection group,26 Severe pneumonia was classified as severe pneumonia group,and 23 cases of sepsis were classified as sepsis group.The children in all 4 groups were tested for serum PCT and CRP levels,and analyzed the value of diagnosing infection and predicting the prognosis of the children.Results With the aggravation of infection,the serum PCT and CRP levels in patients increased significantly(P<0.05);the sensitivity,accuracy and negative predictive value of serum PCT combined with CRP level detection to assess the degree of co-infection in children were higher than that of pure serum Detection of PCT and CRP(P<0.05);the sensitivity,accuracy and negative predictive value of serum PCT combined with CRP level test to assess the degree of co-infection in children were higher than those of simple serum PCT and CRP test(P<0.05).Conclusion Serum PCT combined with CRP level detection can effectively assess whether children with febrile convulsions are complicated with infection,determine the severity of infection,and predict the prognostic effect.
