基于ERAS理念的疼痛护理模式在鼻科局部麻醉手术中的应用

摘　　要：目的:探讨应用ERAS理念的疼痛护理模式在鼻科局部麻醉手术中的效果。方法:选取2019年7月~2020年1月耳鼻咽喉科行鼻科局部麻醉手术90例患者随机分为试验组45例和对照组45例,对照组采用传统的疼痛护理,试验组采用ERAS理念的疼痛护理(包括术前预镇痛护理)实施ERAS疼痛护理小组,并进行相关知识的培训,提高护理人员的整体素质,确保护理的顺利实施。采用视觉模拟量表(VAS)和模糊数字评分法,对患者术中及术后1 h疼痛和舒适度进行评分;并予护理前及护理后对患者疼痛和舒适度、护理满意度和依从性进行评分。结果:试验组中采取术前预镇痛可降低患者术中及术后1 h疼痛;护理前,VAS评分试验组(4.54±2.12)与对照组(4.38±2.21)比较,差异无统计学意义(P>0.05);舒适度评分试验组(2.56±1.96)与对照组(2.68±1.32)比较,差异无统计学意义(P>0.05);护理满意度(88.58%、88.60%)和依从性(85.59%、85.57%)比较,差异无统计学意义(P>0.05);护理后,试验组的VAS评分、舒适度、护理满意度和依从性明显高于对照组,差异有统计学意义(P<0.05)。结论:在鼻科局部麻醉手术的术前预镇痛采取改良式疼痛护理有助于提高患者的护理满意度和依从性,对耳鼻咽喉科的优质护理服务起到积极的促进作用。Objective To explore the effect of ERAS′pain nursing model in nasal surgery.Method In July 2019-January 2020 in otolaryngology section lines of the nose section bureau hemp surgery,90 patients were selected and randomly divided into experimental group 45 cases and the control group 45 cases,control group with traditional pain nursing,the concept of the experimental group uses the ERAS pain nursing(including preoperative analgesia nursing)implement ERAS pain nursing team,training the related knowledge,improve the whole quality of nursing staff,ensure the smooth implementation of nursing.The visual analogue scale(VAS)and the fuzzy digital scoring method were used to evaluate the intraoperative and postoperative pain and comfort of the patient.Pain and comfort,nursing satisfaction and compliance were evaluated before and after care.Results Preoperative analgesia was adopted in the experimental group to reduce intraoperative and 1-hour postoperative pain.Before nursing,there was no difference between VAS score test group(4.54±2.12)and control group(4.38±2.21)(P>0.05).There was no difference in comfort score between the experimental group(2.56±1.96)and the control group(2.68+1.32)(P>0.05).There was no difference between nursing satisfaction(88.58%,88.60%)and compliance(85.59%,85.57%)(P>0.05).After nursing,VSA score,comfort,nursing satisfaction and compliance of the experimental group were significantly higher than those of the control group(P<0.05).Conclusion The nasal branch bureau hemp surgery preoperative analgesia in advance to take improved pain nursing helps to improve the nursing satisfaction and compliance of patients,in otolaryngology department of high quality nursing service plays a positive role in promoting.
