黄体酮联合益宫颗粒对药物流产术后阴道出血的临床研究

摘　　要：目的观察黄体酮联合益宫颗粒对药物流产术后阴道出血的临床疗效及安全性。方法将80例在本院进行药物流产术后阴道出血的患者以随机数字表法分为试验组(40例)和对照组(40例)。2组患者均给予黄体酮胶囊50 mg,每天3次;试验组加用益宫颗粒10 g(开水冲服),每天3次,2组患者均治疗10 d。比较治疗前后2组患者的临床疗效、阴道出血时间、阴道出血量、月经恢复时间、子宫内膜厚度、血清雌二醇(E2)、孕酮(P)、人绒毛膜促性腺激素(β-hCG)水平以及血液流变学指标变化,观察治疗过程中不良反应的发生情况。结果试验组和对照组的总有效率分别为92.50%和75.00%;阴道出血量分别为(64.28±19.85)和(45.72±10.23)mL;子宫内膜厚度分别为(6.73±0.82)和(5.11±0.46)mm;阴道出血时间分别为(4.68±1.02)和(6.35±1.60)d;月经恢复时间分别为(40.01±3.27)和(48.52±3.05)d。上述指标,试验组与对照组比较,差异均有统计学意义(均P<0.05)。试验组和对照组的不良反应发生率为10.00%和20.00%,差异无统计学意义(P>0.05)。结论黄体酮联合益宫颗粒可有效治疗药物流产术后阴道出血,改善激素水平和血液流变学指标。Objective To observe the clinical efficacy and safety of progesterone combined with Yigong granule in the treatment of vaginal bleeding after drug abortion.Methods 80 patients with vaginal bleeding after drug abortion in our hospital were randomly divided into experimental group(40 cases)and control group(40 cases).Both groups were given progesterone capsule 50 mg 3 times a day;the experimental group was treated with Yigong granule 10 g(boiled water),3 times a day.Both groups were treated for 10 days.The clinical efficacy,vaginal bleeding time,vaginal bleeding volume,menstrual recovery time,serum estradiol(E2),progesterone(P)and human chorionic gonadotropin-β(β-HCG)before and after treatment were compared between the two groups level,changes of hemorheological indexes and occurrence of adverse reactions during treatment.Results The total effective rate of the experimental group and the control group were 92.50%and 75.00%;the amount of vaginal bleeding were(64.28±19.85)and(45.72±10.23)mL;endometrial thickness were(6.73±0.82)and(5.11±0.46)mm;the vaginal bleeding time were(4.68±1.02)and(6.35±1.60)d;the menstrual recovery time were(40.01±3.27)and(48.52±3.05)d,respectively.For the above indicators,the difference between the experimental group and the control group were statistically significant(all P<0.05).The incidence of adverse reactions between the experimental group and the control group was 10.00%and 20.00%,without significant difference(P>0.05).Conclusion Progesterone combined with Yigong granule can effectively treat vaginal bleeding after medical abortion,improve hormone level and hemorheology index.
