超分子水杨酸联合点阵激光治疗雄激素性脱发疗效研究

摘　　要：目的观察超分子水杨酸联合点阵激光治疗雄激素性脱发的临床疗效及安全性。方法选择75例雄激素性脱发患者,按照随机表发分为5组,每组15例,分别为水杨酸激光联合组,水杨酸组,酮康唑激光联合组,酮康唑组和激光组。水杨酸激光联合组外用超分子水杨酸洗发露联合2940nm非剥脱点阵铒玻璃激光,酮康唑激光联合组外用酮康唑洗发露联合2940nm非剥脱点阵铒玻璃激光。治疗后由受试者自身评价头皮瘙痒、油腻、鳞屑、脱发症状改善情况,并采用照相机、毛发镜记录受试者脱发区头发生长状况,评价临床疗效。结果治疗后,水杨酸激光联合组总体毛发数量增多,毳毛比例明显下降,并有统计学差异。水杨酸组、酮康唑激光联合组、酮康唑组和激光组毛发数量及毳毛比例无统计学差异。水杨酸激光联合组、水杨酸组可有效改善患者头皮瘙痒、油腻、鳞屑症状,酮康唑激光联合组、酮康唑组对患者头皮瘙痒、鳞屑情况有明显改善。五组患者均未出现头皮刺痛、红斑、瘢痕或感染,未出现血尿常规、肝肾功能异常。结论超分子水杨酸联合点阵激光疗法对雄激素源性脱发患者具有一定的生发效果,并能有效改善脱发患者头皮瘙痒、油腻、鳞屑症状,是改善雄激素源性脱发患者症状的有效辅助手段。Objective To introduce the clinical effect and safety of supramolecular salicylic acid combined with 2940nm nonstripping erbium glass laser in the treatment of androgenic alopecia.Methods Seventy-five patients with androgenetic alopecia(AGA)were randomly divided into 5 groups with 15 patients in each group,namely salicylic acid combination group,salicylic acid group,ketoconazole combination group,ketoconazole group and laser group.Patients in salicylic acid combination group used externally supramolecular salicylic acid shampoo combined with 2940nm non-stripping erbium glass laser.Furthermore,ketoconazole shampoo combined with 2940nm non-stripping erbium glass laser was applied to the scalp of patients in ketoconazole combination group externally.After treatment,patients were required to evaluate the improvement of itch,greasiness,scales,alopecia.Cameras and trichoscopy were used to record the hair growth of patients with AGA to evaluate the clinical effect.Results After treatment,the number of hair increased and the proportion of vellus hairs decreased significantly in the salicylic acid combination group.There was no significant difference in hair number and vellus hair proportion in the other four groups.The salicylic acid combination group and the salicylic acid group could effectively improve the symptoms of itch,greasiness and scales,while the ketoconazole combination group and the ketoconazole group could significantly improve the itch and scales.No scalp tingling,erythema,scar or infection occurred in the 5 groups,and no blood routine,urine routine,liver and kidney function abnormalities occurred.Conclusion Supramolecular salicylic acid combined with 2940nm non-stripping erbium glass laser has a certain effect on hair growth in patients with AGA,and can effectively improve the symptoms of itch,greasiness,scales,which is an effective assistant method to improve the symptoms of AGA.
