壮骨片联合关节镜清理术治疗膝骨关节炎的临床分析

摘　　要：目的探讨壮骨片联合关节镜清理术治疗膝骨关节炎的临床效果。方法将沈阳市骨科医院运动医学科2017年1月—2019年1月收治的236例膝骨关节炎患者随机分为观察组与对照组,每组118例。对照组进行关节镜清理术及常规治疗,观察组在对照组治疗基础上口服壮骨片每日2次,每次4片,持续1个月。治疗前及治疗后1个月、6个月采用西安大略和麦克马斯特大学骨关节炎指数可视化量表(Western ontario macmaster university osteoarthritis index visualization scale,WOMAC)评价患者关节功能,并检测患者关节液中白介素6(Interleukin6,IL-6)及血清降钙素水平。结果观察组患者治疗后1个月、6个月的WOMAC评分、关节积液中IL-6水平均低于对照组,血清降钙素水平均高于对照组差,差异有统计学意义(P<0.05)。结论壮骨片联合关节镜清理术能够改善膝骨关节炎患者的关节功能,可以降低炎症因子的水平、提高保护因子的水平。Objective To investigate the clinical effect of Zhuanggu tablets combined with arthroscopic debridement in the treatment of knee osteoarthritis.Methods A total of 236 patients with knee osteoarthritis from the department of sports medicine of Shenyang Orthopedic Hospital from January 2017 to January 2019 were randomly divided into observation group and control group with 118 cases in each.The control group underwent arthroscopic debridement and conventional treatment,and the observation group took orally Zhuanggu tablets 2 times/d,4 tablets/time on the basis of the treatment of the control group for 1 month.Before treatment and 1 month and 6 months after treatment,the Western Ontario McMaster University Osteoarthritis Index Visualization Scale(WOMAC)was used to evaluate the joint function of the patient,and check the level of IL-6 and serum calcitonin in the joint fluid of the patient.Results The WOMAC score and the level of IL-6 in synovial fluid of the observation group were lower than those of the control group at 1 and 6 months after treatment,the difference was statistically significant(P<0.05),while the serum calcitonin level was higher The difference in the control group was statistically significant(P<0.05).Conclusion Zhuanggu tablets combined with arthroscopic debridement can improve the joint function of patients with knee osteoarthritis,reduce the level of inflammatory factors and increase the level of protective factors.
