史学教授扶正健脾法辅助治疗小儿恶性实体瘤经验探析

摘　　要：史学教授从事小儿恶性实体瘤的中医临床研究十余年,积累了丰富的临床经验。史教授根据儿童恶性实体瘤的特点,结合儿童的体质特点,对于术后、化疗后的恶性实体瘤患儿出现的气血亏虚、血不循经、脾胃失和等证候,以扶正为根本治则,健脾益气养血,旨在尽快恢复脏腑气血的正常状态,从而保证治疗的顺利进行。扶正健脾法的处方多为寒温并用,阴阳同调,气血双补,补而不滞,具有儿科特色。同时史教授中西医互参,注重分期治疗,配合西医不同治疗阶段,调整中医治疗的侧重点,对儿童个体化治疗标准的制定也做出了初步的尝试。在扶正健脾的同时,史教授特别关注患儿的心理健康,应用中医药结合心理疏导改善患儿的心理状态和社会关系,属于中医“调神”的范畴。最后结合典型案例,体现中医扶正健脾法在小儿恶性实体瘤治疗中的效果,为小儿恶性实体瘤的中西医结合治疗提供良好示范。Professor Shi Xue has been engaged in clinical research of malignant solid tumor in children for more than ten years,accumulated rich clinical experience.According to the characteristics of children malignant solid tumors and children physical characteristics,for children after surgery or chemotherapy,the syndromes as deficiency of qi and blood,escaping of blood from vessels and channels,spleen-stomach disharmony.Professor Shi established supporting healthy energy as fundamental therapeutic principle,to invigorate the spleen,nourish qi and nourish blood,aims to recover as soon as possible,so as to ensure the smooth progress of treatment.The prescriptions of strengthening the spleen method mostly are combination of cold-warm,harmonization of yin and yang,qi-blood reinforcing,supplement but not stagnation,with pediatric characteristics.At the same time,Professor Shi pays attention to integrate of traditional Chinese and western medicine,staged treatment,adjusting the emphasis of traditional Chinese medicine treatment coordinating with treatment stages of western medicine.Professor Shi also made preliminary attempt to establish the standard of individualized treatment for children.Additionally,Professor Shi pays special attention to the mental health of children,applies psychological counseling and herbs to improve the psychological state and social relations,which belongs to the category of“regulating the spirit”of traditional Chinese medicine.Finally,combined with typical cases,it reflects the effect of strengthening the spleen in the treatment of pediatric malignant solid tumor,and provides a good demonstration for the treatment of pediatric malignant solid tumor with integrated Chinese and Western medicine.
