微血管减压术与经皮穿刺球囊/微球囊压迫术治疗原发性三叉神经痛的疗效观察

摘　　要：目的探讨微血管减压术(MVD)与经皮穿刺球囊/微球囊压迫术(PBC/PMC)两种术式治疗原发性三叉神经痛(PTN)的疗效。方法回顾性分析2019年1月至2020年1月在我院进行治疗的220例PTN患者临床资料,依据治疗方式不同分为MVD组(n=62)与PBC/PMC术组(n=158),比较两种术式在PTN患者中的疗效。结果两组治疗有效率比较差异无统计学意义(P>0.05);MVD组住院费用明显高于PBC/PMC术组,住院时间、手术时间均长于PBC/PMC术组(P<0.05);随访12个月MVD组术后并发症发生率及复发率明显低于PBC/PMC术组(P<0.05)。结论MVD与PBC/PMC两种术式治疗PTN疗效均较佳,但前者复发率及并发症发生率均更低,而PBC/PMC则具有安全性高、创伤小、手术时间及住院时间短的优势,临床可根据患者的需求给予针对性治疗。Objective To investigate the curative effect of microvascular decompression(MVD)and percutaneous balloon compression/percutaneous micro-balloon compression(PBC/PMC)in the treatment of primary trigeminal neuralgia(PTN).Methods Clinical data of 220 patients with PTN treated in our hospital between January 2019 and January 2020 was retrospectively analyzed.According to the treatment methods,the patients were divided into MVD group(n=62)and PBC/PMC group(n=158).Therapeutic effects of the two methods were compared.Results There was no significant difference in the effective rate of treatment between the two groups(P>0.05).The hospitalization cost,length of hospital stay and operation time of the MVD group were significantly more or longer than those of the PBC/PMC group(P<0.05).After 12 months of follow-up,it was found that the incidence of complications and recurrence rate in the MVD group were significantly lower than those in the PBC/PMC group(P<0.05).Conclusion Both MVD and PBC/PMC are effective in the treatment of PTN.However,the former has a lower recurrence rate and a lower incidence of complications.PBC/PMC has the advantages of high safety,small trauma,short duration,and short hospital stay.Clinically,the targeted treatment can be chosen according to the patient’s needs.
