微型静脉皮瓣与微型邻指皮瓣治疗伴有皮肤软组织缺损的手指末节离断再植的疗效及安全性分析

摘　　要：目的探讨微型静脉皮瓣与微型邻指皮瓣治疗伴有皮肤软组织缺损的手指末节离断再植的疗效及安全性。方法选取我院105例(105指)伴有皮肤软组织缺损的手指末节离断患者,采用随机数字表法分为观察组(n=53)和对照组(n=52),对照组采用微型邻指皮瓣治疗,观察组采用微型静脉皮瓣治疗。比较两组伤指运动功能、伤指恢复情况、伤指血液循环情况、并发症发生情况。结果观察组伤指运动功能优良率及血液循环优良率高于对照组;两点辨别觉小于对照组,远侧指间关节活动度、指甲长度高于对照组;并发症发生率低于对照组(P<0.05)。结论微型静脉皮瓣治疗对皮肤软组织缺损手指末节离断再植患者的疗效优于微型邻指皮瓣治疗,能有效改善伤指运动功能及血液循环情况,促进伤指术后恢复,减少并发症,具有较高的安全性。Objective To explore the effect and safety of mini-venous flap and mini-adjacent finger flap in the treatment of replantation of severed finger with skin and soft tissue defects.Methods A total of 105 patients(105 fingers)with skin and soft tissue defects and severed distal fingers in our hospital were enrolled.They were divided into an observation group(n=53)and a control group(n=52)by random number table method.The control group was treated with mini-adjacent finger flap,while the observation group was treated with mini-venous flap.The motor function,recovery and blood circulation of injured fingers,and the occurrence of complications were compared between the two groups.Results The good rate of injured fingers’motor function and blood circulation in the observation group was higher than that in the control group.The two-point discrimination was lower and range of motion and nail length of distal interphalangeal joints were higher in the observation group than those in the control group.The incidence of complications was lower in the observation group than that in the control group.All differences were statistically significant(P<0.05).Conclusion Compared with mini-adjacent finger flap,curative effect of mini-venous flap is better for severed distal finger replantation in patients with skin and soft tissue defects.It can effectively improve the motor function and blood circulation of injured fingers,promote the postoperative recovery of injured fingers,and reduce the complications.It also has a high safety.
